Abstract/Summary:
Objective:Through the detection of rheumatoid arthritis(rheumatoid arthritis,RA)patients’ periodontal condition and serum rheumatoid factor(RF),anti-cyclic citrullinated peptide antibodies(anti-cyclic citrullinated peptide antibodies,anti-CCP antibodies),anti-heart Phospholipid antibody(anti-cardiolipin antibody,ACA)level,analyze the relationship between the periodontal status of patients with rheumatoid arthritis and RF,anti-CCP antibody,ACA.It aims to provide clinical evidence for the study of the relationship between periodontitis and rheumatoid arthritis,and to provide new ideas for the prevention and treatment of periodontitis and rheumatoid arthritis.Methods:From October 2018 to December 2020,among the outpatient and hospitalized rheumatoid arthritis patients diagnosed for the first time by the Department of Rheumatology and Immunology of Shanxi Provincial People’s Hospital,200 cases were randomly selected as research subjects according to the principle of voluntariness.)Methods The serum rheumatoid factor and anti-cyclic citrulline antibody levels of the subjects were detected,and the serum anticardiolipin antibody Ig A(ACA-Ig A),Ig G(ACA-Ig G),and Ig M(ACA-Ig M)levels were detected by chemiluminescence.Periodontal examination was given,and bleeding index(BI),clinical attachment loss(AL),and probing depth(PD)were recorded.According to the periodontal status,the study subjects were divided into two groups of patients with periodontal healthy RA and RA with periodontitis.According to the degree of periodontitis,the patients with RA with periodontitis were divided into three groups:mild,moderate and severe periodontitis.(1)Compare the levels of serum RF,anti-CCP antibody and ACA in patients with periodontal healthy RA and RA patients with periodontitis,and analyze the influence of the periodontal status of RA patients on the serum RF,anti-CCP antibody and ACA levels.(2)Compare the serum RF,anti-CCP antibody and ACA levels of the three groups of RA patients with mild,moderate and severe periodontitis,and analyze the effects of different degrees of periodontitis on the serum RF,anti-CCP antibody and ACA levels of RA patients.(3)Analyze the relationship between the periodontal index and serum ACA,anti-CCP antibody and RF levels in RA patients.Results:1.Analysis of periodontal status in patients with rheumatoid arthritis:Among the 200 RA patients,16 were periodontal healthy(8%);184 were periodontitis(92%),of which 48 were mild periodontitis(26%);and 91 were moderate periodontitis(49.5%);45 cases of severe periodontitis(24.5%).2.Comparison of general information of each group:2.Comparison of general information of each group:There was no significant difference in gender and age between the two groups of patients with periodontal healthy RA and RA with periodontitis(P>0.05).There was no significant difference in age and gender among the three groups of RA patients with mild,moderate and severe periodontitis(P>0.05).3.Comparison of serum RF,anti-CCP antibody and ACA levels between the two groups of patients with periodontal healthy RA and RA with periodontitis:The serum ACA-Ig G,ACA-Ig M,and anti-CCP antibody levels of RA patients with periodontitis were higher than those of periodontal healthy RA patients,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the ACA-Ig A and RF levels between the two groups(P>0.05).4.Comparison of serum RF,anti-CCP antibody and ACA levels in the three groups of RA patients with mild,moderate and severe periodontitis:Comparison of serum anti-CCP antibody,ACA-Ig G and ACA-Ig M levels in the three groups of RA patients with mild,moderate and severe periodontitis,the difference was statistically significant(P<0.05),the RA group with severe periodontitis> with moderate Periodontitis RA group>with mild periodontitis RA group,while the comparison of serum RF and ACA-Ig A levels among the three groups was not statistically significant(P>0.05).5.Correlation analysis of periodontal status and serum ACA and anti-CCP antibody levels in patients with rheumatoid arthritis:Serum ACA-Ig G levels in RA patients were positively correlated with AL,PD,and BI(the correlation coefficients rs were 0.362,0.203,0.171,P<0.05).Serum ACA-Ig M levels were positively correlated with AL and PD(correlation coefficients rs were 0.404 and 0.313,P<0.05),but there was no correlation with BI(P>0.05).Serum ACA-Ig A levels have no correlation with AL,PD,BI(P>0.05).Serum anti-CCP antibody levels were positively correlated with AL,PD,and BI(correlation coefficients rs were 0.352,0.303,0.215,P<0.05).Conclusion:The prevalence of periodontitis was higher in RA patients,among which moderate to severe periodontitis was higher.Degree of periodontal inflammation of patients with RA serum ACA,anti CCP antibody levels,and after adjustment for confounding factors including age,gender,periodontal destruction and RA patients serum ACA,anti CCP antibody levels were positively correlated relationship,patients with periodontitis can cause elevated levels of serum anti CCP antibody,ACA,suggesting periodontitis with RA may exist a certain relationship,Periodontitis may affect the progression and prognosis of RA.This study provides clinical evidence for the study of the relationship between the two,and provides a new idea for the prevention and treatment of periodontitis and RA.
