| Objective:To compare the effect of quadriceps tendon and bone–patellar tendon–bone autograft on anterior cruciate ligament reconstruction.Methods:In PubMed,EMbase,the Cochrane library,a database of Wanfang and Cnki database on literature retrieval,the quadriceps tendon and published are determined,bone patellar tendon-bone autograft controlled clinical studies,from building library retrieval time on May 31,2019,according to include and exclude standard literature screening and quality evaluation and data extraction,Review Manager 5.3 statistical software was used to conduct meta-analysis on the difference of forward displacement between the Knee and the Knee,Lachman test,axial shift test,Lysholm score,International Knee Documentation committee(IKDC)objective grade,anterior knee pain and transplant failure rate.Results:A total of 6 studies were included,including 915 anterior cruciate ligament reconstructions,495 quadriceps tendon autografts and 420 bone–patellar tendon–bone autografts.The results of meta-analysis showed : The difference between quadriceps tendon and bone–patellar tendon–bone autograft in the anterior displacement of tibia was less than3 mm [OR=1.53,95% CI(0.68,3.44)P=0.31],3-5mm[OR=0.64,95% CI(0.31,1.35)P=0.24],greater than 5mm[OR=1.18,95% CI(0.33,4.22)P=0.80],and the negative rate of Lachman test [OR=0.88,95% CI(0.38,2.02)P= 0.76].The negative rate of axial shift test [OR=0.63,95% CI(0.24,1.68)P =0.36] showed no significant difference.Furthermore,there was no significant difference in Lyshlom score [OR=-0.14,95% CI(-2.62,2.35)P =0.92],IKDC objective grade [OR=0.87,95% CI(0.47,1.60)P =0.66] and transplant failure rate [OR=0.76,95% CI(0.28,2.02)P =0.58].However,autograft of quadriceps tendon was superior to bone–patellar tendon–bone autograft in reducing anterior knee pain [OR=0.16,95% CI(0.09,0.29)P<0.00001].Conclusion:Clinical and functional outcomes and graft survival rate for quadriceps tendon autograft were comparable with bone–patellar tendon–bone autografts.However,quadriceps tendon autograft had significantly less anterior knee pain compared with bone–patellar tendon–bone autograft,quadriceps tendon autograft is certainly a suitable alternative graft choice for patients undergoing anterior cruciate ligament reconstruction. |
