Abstract/Summary:
BackgroundPolycystic ovarian syndrome is a widespread reproductive endocrine disease that affects at least 10% of young women.Its clinical manifestations are diverse,and the pathogenesis is related to many factors,which seriously affects the physical and mental health of women of childbearing age.Vitamin D levels in PCOS patients have been found to be deficient,and chronic low-level inflammation is found to be an important factor in the disease.The study provides guidance for future clinical treatment by detecting the expression of vitamin D and C-reactive protein levels in PCOS patients and analyzing their clinical significance.ObjectiveTo study the expression of vitamin D and C-reactive protein in vivo by detecting the serum of patients with polycystic ovary syndrome(PCOS)and healthy people;to analyze the correlation between serum vitamin D,C-reactive protein and other indicators in patients with PCOS;to analyze the correlation between serum vitamin D,C-reactive protein and other indicators in patients with PCOS,obesity group and non obesity group The correlation of serum vitamin D,C-reactive protein and other indicators between insulin resistance group and non insulin resistance group.MethodsFrom October 2018 to October 2019,patients with polycystic ovary syndrome who were diagnosed in the First Affiliated Hospital of Xinxiang Medical University were selected.According to the inclusion criteria and exclusion criteria,43 female patients with polycystic ovary syndrome,aged 16-36 years old,were selected from the experimental group,with an average age of(25.48±4.99 years old).40 healthy women in the same period were selected as the control group,aged 19-37 years,with an average age of(26.67±4.31 years old).Fasting venous blood was collected from all subjects within 2-4 days of menstruation(natural cycle)(amenorrhea patients need to stop menstruation for 3 months or more)and 8-10 am,and their age,height and weight were recorded.Six items of sex hormones(E2,FSH,LH,PRL,PROG,TES),Vit D,CRP,FPG and FINS were measured respectively.Insulin resistance and body mass index were calculated according to the formula.Finally,SPSS 25.0 was used for statistical analysis according to the results.Results1.There was no significant difference in age,PROG between PCOS group and healthy people(P > 0.05),but vitamin D in PCOS group was lower than that in control group(P < 0.05),E2,CRP,FSH,LH,PRL,TES,FPG,FINS and HOMA-IR in PCOS group were significantly higher than those in control group(P < 0.05).2.43 cases of experimental group were divided into two groups according to BMI ≥ 25,and PCOS patients were divided into obese group and non obese group.The differences between the two groups were compared.The results showed that the obese group accounted for 44% of the total number of PCOS patients,while the non obese group accounted for 56%.Compared with each index between the two groups,the expression level of Vit D in the obese group was significantly lower than that in the non obese group,and the levels of CRP,LH,FPG,FINS The expression levels of HOMA-IR and BMI in obesity group were significantly higher than those in non obesity group(P < 0.05);There was no significant difference in the expression levels of E2,FSH,PRL,PROG and TES between the two groups(P > 0.05).3.43 cases of experimental group were divided into two groups,(HOMA-IR)≥2.69 was divided into insulin resistance group,(HOMA-IR)< 2.69 was non insulin resistance group,research and analysis found that IR group accounted for 65% of the total number of PCOS patients,non IR group accounted for 35% of the total number,compared the correlation between the two groups,Vit D in insulin resistance group.CRP,FPG,FINS,HPMA-IR,BMI were significantly higher than non insulin resistance group,the difference was statistically significant(P < 0.05),but E2,FSH,LH,PRL,PROG,TES two groups were not statistically significant(P > 0.05).4.Pearson correlation analysis of vitamin D and other indicators in PCOS group showed that serum vitamin D was negatively correlated with CRP,E2,TES,FPG,FINS HOMA-IR and BMI(P < 0.05),and the differences were statistically significant(P < 0.05).There was no significant difference between serum vitamin D and FSH,LH,PRL and PROG(P > 0.05).Conclusions1.This study found that the level of vitamin D in patients with polycystic ovary syndrome was lower than that in normal people,and the level of C-reactive protein was higher than that in normal people.There was insulin resistance in patients with polycystic ovary syndrome.2.In patients with polycystic ovary syndrome,vitamin D deficiency,chronic inflammation and insulin resistance in obese group were more serious than those in non obese group;In insulin resistance group,vitamin D deficiency,chronic inflammation and obesity were more serious.3.In patients with polycystic ovary syndrome,vitamin D is negatively correlated with C-reactive protein,obesity and insulin resistance.
