Abstract/Summary:
Objective: Analyze the related pathogenic factors of lactation mastitis(LM)and non-puerperal mastitis(NPM),and perform corresponding prevention and treatment after identifying the pathogenic factors,which can effectively reduce the incidence of mastitis.And to provide references and references for further research on the cause,diagnosis,preventive measures and clinical treatment of mastitis.Methods: This study collected the medical records of 50 lactating mastitis patients and 50 non-puerperal mastitis patients from the Department of Breast and Thyroid Surgery of Qinghai University Affiliated Hospital from January 2019 to December 2020,and excluded other diseases that affect this study.The physical examination center of the Affiliated Hospital of Qinghai University,with informed consent,voluntarily participated in the questionnaire survey of this study.50 women who did not suffer from mastitis during breastfeeding were selected as the control group for breast-feeding mastitis;the breast surgery specialist examination and breast imaging examination showed that there were no abnormal breast nodules and50 healthy women with obvious duct dilatation were used as a control group for non-puerperal mastitis.We collect general data of patients and women in the physical examination center,including age,height,weight,living habits,breastfeeding history,menstrual history,reproductive history,and history of past diseases,etc.Relevant research factors were included and the observation indicators were clarified.The clinical factors included in the lactation mastitis group were:birth age,lactation time,whether breast stasis occurred repeatedly during breastfeeding,nipple cleaning after breastfeeding,whether nipples were damaged,and whether breast surgery was performed History,anxiety and smoking history,etc.The clinical research factors included in the non-puerperal mastitis group include: age,body mass index,number of births,age at first childbirth,age at menarche,smoking,drinking,and inverted nipple.Use SPSS25.0 statistical software to perform statistical analysis on the collected data,use single factor analysis statistically significant factors as variables,and perform multivariate regression analysis.Results: The results of a case-control analysis of the LM group: There were statistically significant differences between the two groups of subjects in milk stasis,nipple rupture,breast trauma surgery history,breastfeeding time,and nipple cleaning(P<0.05).However,there were no statistically significant differences in age at childbirth,smoking,and anxiety(P>0.05).Logistic multivariate analysis showed that: repeated breast stasis,ruptured nipple,and history of breast trauma surgery during breastfeeding are independent risk factors for breast-feeding mastitis;frequent nipple cleaning and breastfeeding time>6 months were protective factors.NPM group case-control analysis results: The two groups of subjects had statistically significant differences in body mass index,number of births,age at menarche,age at first childbirth,and inverted nipples(P<0.05).However,there was no significant difference in age,active smoking,passive smoking and drinking history(P>0.05).Logistic multivariate analysis showed that inverted nipples,overweight and obesity,frequent births,and delayed menarche are independent risk factors for non-puerperal mastitis;and late first childbirth was a protective factor.Conclusion: The main pathogenic factors of breast-feeding mastitis are nipple rupture,repeated milk stasis during breast-feeding,and history of breast trauma surgery;wash nipples after breastfeeding and breast-feeding time>6 months can reduce the risk of the disease.The main pathogenic factors of non-puerperal mastitis are overweight and obesity,frequent births,delayed menarche and inverted nipples;late first childbirth can reduce the risk of the disease.
