Study On The Correlation Between NLR,PLR And Left Ventricular Hypertrophy In Maintenance Hemodialysis Patients

Objective: To explore the correlation between NLR,PLR and LVH in MHD patients,and whether NLR and PLR can be used as predictors of LVH in MHD patients.Methods: Selected 138 ESRD patients who received MHD treatment at the Blood Purification Center of the Nephrology Department of Yan’an University Affiliated Hospital from January 2015 to December 2020.All patients received HD treatment 3times a week for 4 hours each time.Collect general data such as age,gender,dialysis age,and primary disease of all selected patients,and collect WBC、NEU、LYM、PLT、MONO、Hb、ALB、TC、TG、HDL-C、LDL-C、Urea、Cr、UA、Ca、P、hs CRP and other laboratory indicators.The ratio of NEU to LYM calculates NLR,and the ratio of PLT to LYM calculates PLR.Collection of ultrasound examination results include:LVEDD、LVPWT、IVST.According to LVMI male ≥115 g/m2 and female ≥95 g/m2,138 patients were divided into LVH group and non-LVH group,and the differences in indicators between the two groups were compared.Logistic regression was used to analyze the risk factors of LVH and the ROC curve was used.Determine the cut-off value of NLR and PLR,as well as the sensitivity and specificity of predicting LVH,P<0.05 indicates that the difference is statistically significant.Results:(1)Compared with patients in the non-LVH group,the age、NEU、TC、UA、P、hs CRP、NLR、PLR of the LVH group were higher(P<0.05),and ALB was lower(P<0.05);(2)Multivariate logistic regression analysis showed:NLR(OR=1.944,95%CI:1.259-3.000,P=0.003),PLR(OR=1.034,95%CI:1.017-1.051,P<0.001)are the occurrence of LVH in MHD patients,ALB(OR=0.887,95%CI:0.799-0.985,P=0.025)is a protective factor that affects the occurrence of LVH;(3)ROC curve analysis shows that NLR predicts the AUC of LVH in MHD patients is 0.785(95%CI:0.706-0.864,P<0.001).When the NLR takes 4.42 as the prediction cut point,the sensitivity is 82.4% and the specificity is 67.9%.The AUC of PLR is 0.818(95%CI:0.744-0.892,P<0.001).When PLR is 145.13 as the prediction cut point,the sensitivity is 89.4% and the specificity is 67.9%.The AUC of NLR+PLR is 0.869(95%CI:0.803-0.935,P<0.001),which shows that the predictive value is higher when combined.Conclusions:(1)The NLR、PLR of MHD patients are related to LVH,and elevated NLR and PLR are independent risk factors for LVH,ALB is a protective factor;(2)The combined detection of NLR and PLR has certain predictive value for LVH of MHD patients.
