| Objective: By comparing the short-term clinical efficacy of microendoscopic minimally invasive lumbar interbody fusion(MED-TLIF)and transforaminal lumbar interbody fusion(TLIF)in the treatment of lumbar degenerative diseases,to verify whether microendoscopic minimally invasive lumbar interbody fusion has better efficacy in the treatment of such diseases,so as to provide a more minimally invasive and safer surgical method for the treatment of lumbar degenerative diseases.Methods: From January 2020 to October 2020,36 patients with lumbar degenerative diseases admitted to the Department of Spinal Surgery of the Wuhan No.4 Hospital were selected,including 17 males and 23 females,aged between 46-75 years old.The patients were divided into experimental group and control group according to their operation willingness.The experimental group was treated with MED-TLIF,and the control group was treated with open TLIF.The gender,age,visual analogue score(VAS),Oswestry disability index(ODI)and postoperative drainage volume of the two groups before operation,three-days,one-month and three-months after operation and operation time,intraoperative blood loss and intraoperative fluoroscopy times of the two different operation methods were recorded and analyzed statistically.Results: 1.40 patients were followed up for 3 months.There was no significant statistically difference in gender,age,lesion type between the two groups(P > 0.05).2.The operation data of the two groups were statistically compared: the operation time and intraoperative fluoroscopy times of MED-TLIF group were significantly more than those of open TLIF group(P < 0.05),but the intraoperative blood loss and postoperative drainage volume of MED-TLIF group were significantly lower than those of open TLIF group(P < 0.05).The differences between them was statistically significant.3.Visual analogue score(VAS)and Oswestry disability index(ODI)as indexes of curative effect were compared: the VAS and ODI of the two groups after operation were significantly lower than those before operation(P < 0.05),and there was no significant difference in the VAS and ODI between the two groups before operation(P >0.05),which was comparable.The score of MED-TLIF group was lower than that of open TLIF group(P< 0.05)at 3 days and 1 month after operation.There was no significant difference in the score between the two groups(P > 0.05)at three months after operation.Conclusion: Two kinds of surgical methods have good therapeutic effect for lumbar degenerative diseases.Compared with traditional open TLIF,MED-TLIF has the advantages of less trauma and faster postoperative recovery,but it significantly increases the operation time and the intraoperative fluoroscopy times.There was no significant difference in the medium term(3 months after operation)recovery effect between the two kinds of surgical methods.In addition,due to the short follow-up time in this study,it should be temporarily with cautious attitude whether the long-term complication rate of MED-TLIF will rise. |
