【Objective】The results of functional recovery after surgical reconstruction of traumatic peripheral nerve injury are not satisfactory,so it is necessary to find an effective auxiliary treatment.Low-intensity pulsed ultrasound has been shown to promote the regeneration and functional recovery of sciatic nerve injury by promoting the expression of nutrient factors.4-aminopyridine as a potassium channel blocker has also been shown to effectively promote regeneration and functional recovery of sciatic nerve injury.In this study,low-intensity pulsed ultrasound and 4-aminopyridine were combined in a rat model of sciatic nerve injury to evaluate the therapeutic effect of the combination on regeneration of sciatic nerve injury.【Method】1.125 SD rats were randomly divided into 5 groups,25 for each group: sham operation group;Sciatic nerve injury group;Low intensity pulsed ultrasound group;4-aminopyridine group;Low intensity pulsed ultrasound + 4-aminopyridine group;2.At 1 day,1,2,4 and 8 weeks after the operation,the motor function of rats in each group was evaluated by the measurement of sciatic nerve function index.3.At 1,2,4 and 8 weeks after the operation,the target muscle atrophy of sciatic nerve was evaluated by the measurement of the wet weight ratio of gastrocnemius muscle.4.HE staining was used to evaluate the regeneration of sciatic nerve at 4 and 8 weeks after surgery.5.MASSON stanning was used to evaluate the formation of anastomotic scar.5.The expression of IL-10 in sciatic nerve was detected by Western blot and PCR at 1 week after surgery.【Result】1.The result of sciatic nerve function index showed that the sciatic nerve function index of the LIPUS group,the 4 – AP group and the LIPUS + 4 – AP group were higher than in the SNI group,and the LIPUS+ 4-AP group was higher than the 4-AP group and the LIPUS group at 4 and 8 weeks after the surgery.And the differences were statistically significant.2.The results of wet weight rate of gastrocnemius muscle indicated that with the development of time,the gastrocnemius muscle gradually atrophied,and the degree of atrophy reached the heaviest at a certain point between 4weeks and 8 weeks after the surgery,then began to recover gradually.The wet weight rate of gastrocnemius muscle in the LIPUS group,the 4 – AP group and the LIPUS + 4 – AP group were higher than in the SNI group,and the LIPUS+ 4-AP group was the highest at 4 and 8 weeks after the surgery.And the differences were statistically significant.3.The results of HE stanning showed that the medullated nerve fibers area of the LIPUS group,the 4 – AP group and the LIPUS + 4 – AP group were higher than in the SNI group,and the LIPUS+ 4-AP group was the highest and the difference was statistically significant at 4 and 8 weeks after the surgery.4.The results of MASSON stanning showed that about the scar area in nerve anastomosis of rats,there was no significant difference among the SNI group,the 4-AP group,the LIPUS group and the LIPUS+4-AP group.5.The expression of IL-10 showed that the expression levels of the LIPUS group and the LIPUS + 4 – AP group were higher than in the SNI group and the difference was statistically significant at 1week after the surgery.The results were consistent at m RNA and protein levels.【Conclusion】1.Low-intensity pulsed ultrasound and 4-aminopyridine were effective in promoting the regeneration and functional recovery of sciatic nerve injury,and the combination of the two treatments could promote the regeneration and functional recovery of sciatic nerve injury better.2.Low intensity pulsed ultrasound and 4-aminopyridine had no significant effect on the formation of nerve scar.3.Low intensity pulsed ultrasound can promote the expression of IL10 and but 4-AP has no significant effect to the expression of IL10.4.The effect that promoting regeneration of sciatic nerve injury of the Low intensity pulsed ultrasound may be related to its effects that promoting the expression of IL-10.
