Objective: Observe the effect of alanylglutamine early nutritional support on liver f unction in patients with sepsis.Methods: The patients with sepsis who were admitted to t he Department of Emergency Medicine of the General Hospital of Tianjin Medical Univer sity from January 2019 to December 2019 were selected according to the European Critic al Medicine Association(ESICM)and the American Society of Critical Medicine(SCCM)jointly released the sepsis 3.0 standard,a total of 33 patients were diagnosed with sepsis or septic shock.Randomly divided the patients into a control group and an observation group.The control group was given conventional treatment.The observation group was co mbined with intravenous alanylglutamine infusion on the basis of conventional treatment.Record the patient’s gender,age,vital signs,infection site,underlying disease and surgical history.10 ml of venous blood was collected on the 1st,3rd and 5th day of admission,a nd the white blood cell(WBC),neutrophil absolute value(NEU #),lymphocyte absolute value(LYM #),procalcitonin were tested(PCT),full-range C-reactive protein(CRP),alani ne aminotransferase(ALT),γ-glutamyl transpeptidase(GGT),total bilirubin(TBIL),according to 24 hours of admission and the 5th day of treatment Clinical data,calculate SOFA score,APACHE II score and NUTRIC score.Use statistical software SPSS20.0 to organize,process and analyze data.The measurement statistics are expressed as mean ± standard deviation(x ± s),repeated measurement variance analysis is used within the group,and t test is used between the groups;Results: In the observation of the treatment of 33 patie nts with sepsis,there was no statistically significant difference in age,gender and infection site between the two groups(P> 0.05).The SOFA score APACHE II score and NUTRI C score were calculated on admission in the two groups,and there was no statistical diff erence(P> 0.05)after the test.The full-range C-reactive protein and procalcitonin decreased(P <0.05).Compared with the control group,the observation group had no significant difference in the absolute values of white blood cells,neutrophils,full-range C-reactive pr otein and procalcitonin(P> 0.05),and the absolute value of lymphocytes increased signific antly(P <0.05).After treatment,the observation group and the control group decreased alanine aminotransferase,γ-glutamyl transpeptidase,and total bilirubin compared with before treatment(P <0.05).Compared with the control group,the observation group decreased the alanine aminotransferase,γ-glutamyl transpeptidase,and total bilirubin more significantly(P <0.05).And after 5 days of continuous treatment,the SOFA score APACHE II score and NUTRIC score were recalculated,both of which were lower than when they were admitted(P <0.05),in which the observation group decreased significantly compared with the control group(P <0.05).Conclusion: The early nutritional support of glutamine can im prove the liver function of patients with sepsis,promote the proliferation of lymphocytes,and relieve the criticality of the patients’ condition.But early nutritional support of glutam ine does not improve the infection.
