Objective: The fundus pictures of patients with diabetic retinopathy were quantitatively analyzed by artificial intelligence technology,the number and area of hemorrhage and exudation in the posterior pole of the retina were calculated,and the changes of retinal thickness under the macular fovea were recorded by optical coherence tomography to evaluate the clinical efficacy of lecithin complex iodine capsule combined with pancreatic kininogenase enteric-coated tablets and Zhixue Quyu Mingmu tablets in the treatment of non-proliferative diabetic retinopathy with macular edema.Methods: 41 cases(47 eyes)of non-proliferative diabetic retinopathy with macular edema treated in the Department of Ophthalmology of Shandong Provincial Hospital from June 2019 to January 2021 were selected as subjects.On the basis of vitreous injection of 0.05 mg Conbercept,the patients were followed up regularly and injected as needed according to the degree of macular edema and visual acuity decline.Control group(20 cases,24 eyes)were treated with Zhixue Quyu Mingmu Tablet and Pancreatic Kininogenase Enteric-coated Tablets.The observation group(21 cases,23 eyes)were treated with Iodized Lecithin Capsules on the basis of treatment in the control group.The best corrected visual acuity,the number and area of retinal hemorrhage and exudation,the thickness of macular fovea and adverse reactions were recorded before treatment and 1,3 and 6 months after treatment.Results: The median(P25 and P75)was used to express.In the control group: The BCVA(Log MAR)of control group before treatment and 6 months after treatment were 0.52(0.33,0.96)and 0.23(0.11,0.79),P<0.01;CMT(um)were 325(297,461)and 240(213,263),there was significant difference in the decrease of CMT,P<0.01.The total bleeding area(mm2)of fundus images was 37.95(26.90,56.63)、15.21(10.97,28.67).The total exudation area(mm2)was 31.47(21.72,63.66)、13.39(7.96,30.58).The maximum bleeding area(mm2)was 4.24(2.82,5.41)、2.88(1.49,3.67).The maximum exudation area(mm2)is 6.28(2.40,9.10)、2.59(1.26,5.07).The total hemorrhage area,total exudation area,maximum bleeding area and maximum exudation area of retina decreased significantly,(P<0.05).The total bleeding area,total exudation area,maximum bleeding area and maximum exudation area of retina decreased significantly.The number of retinal hemorrhage(including microhemangioma)was significantly lower than that before treatment,but there was no significant difference in the number of retinal exudation before and after treatment(P>0.05).In the observation group,BCVA(Log MAR)was significantly improved before and after treatment : 0.60(0.52,1.00)、0.22(0.10,0.40)(P<0.01).CMT(um)were 328(299,398)and 233(211,247),there was significant difference in the decrease of CMT,P<0.01.The total bleeding area(mm2)of fundus images was 47.48(32.91,63.32)、8.41(4.91,12.05).The total exudation area(mm2)was 34.01(22.18,49.36)、8.02(0.88,11.99).The maximum bleeding area(mm2)was 4.45(3.59,5.35)、1.36(1.11,2.03).The maximum exudation area(mm2)is 6.19(3.20,7.84)、1.05(0.62,1.73).And the total hemorrhage area,maximum bleeding area,total exudation area and maximum exudation area of retina were significantly decreased compared with those before treatment,and the difference was statistically significant(P <0.01).The number of retinal microhemangioma or bleeding site and exudate decreased,and the difference was statistically significant(P<0.01).After treatment,there were significant differences in total retinal hemorrhage,exudation area,maximum hemorrhage and exudation area between the two groups(P<0.01).There was no significant difference in the number of retinal microhemangioma,bleeding site and exudate between the two groups.(P>0.05).There was no significant difference in the number of retinal microhemangioma,bleeding site and exudate between the two groups.The times of vitreous injection within half a year,the control group: 3.33 ±0.637,the observation group: 3.35 ±0.487,there was no significant difference between the two groups(P>0.05).Conclusion:Pancreatic Kininogenase Enteric-coated Tablets and ZhixueQuyuMingmu Tablet can effectively reduce or promote absorption of retinal hemorrhage and exudation in the treatment of non-proliferative diabetic retinopathy,thereby improving vision.The two drugs combined with Iodized Lecithin Capsules are more effective It can effectively promote the absorption of retinal hemorrhage,has good safety,and is worthy of clinical application.
