Objective: In recent decades,the prevalence of diabetes in China has been increasing year by year,and the survival time of diabetic patients has been prolonged year by year.Years of diabetes history easily affects the heart,brain,kidney and other important organs.Due to the invisibility of diabetic nephropathy,many patients with diabetic nephropathy have entered the late stage of diabetic nephropathy to the uremic stage of diabetic nephropathy.Most patients with diabetic nephropathy and uremia require long-term hemodialysis maintenance treatment.In order to further extend the long-term survival rate of patients with diabetic nephropathy in the uremic stage,controlling a stable blood glucose concentration is the main responsibility of our clinicians.In addition to good management of patients and medication to control blood glucose concentration,we should also manage the blood glucose concentration of patients with diabetic nephropathy and uremia during hemodialysis.Only a stable blood glucose concentration can improve the survival rate of patients.The purpose of this study is to explore the effect of different concentrations of sugar-containing dialysate for hemodialysis on blood glucose concentration in patients with diabetic nephropathy and uremia,and to choose a more suitable hemodialysis solution with glucose concentration for patients with diabetic nephropathy and uremia,and ultimately improve the patient’s Survival rate.Methods: Collected patients with diabetic nephropathy who underwent hemodialysis in the hemodialysis room of the Jining No 1People’s Hospital from November 2019 to May 2020.40 patients with diabetic nephropathy in the uremic stage were included,and the two groups were recorded separately.Patients’ gender,age,height,weight,fasting blood glucose,glycosylated hemoglobin,hemoglobin,blood creatinine,urea nitrogen,total cholesterol,and plasma albumin levels were randomly divided into sugar-free dialysate group and sugar-containing dialysate group,and used separately Hemodialysis with sugar-free dialysate and sugar-containing dialysate was performed for 4hours each time,3 times a week,and the observation period was 12 weeks.The blood glucose values of the two groups of patients were recorded at 0h,2h,and 4h each time hemodialysis,using SPSS 26.0Statistical software for processing and analysis,P<0.05 is statistically significant.Results:1.There was no statistical difference in gender,age,height,weight,fasting blood glucose,glycosylated hemoglobin,hemoglobin,blood creatinine,urea nitrogen,total cholesterol,and plasma albumin levels between the sugar-free dialysis group and the sugar-containing dialysis group(P> 0.05).2.With the prolongation of hemodialysis time,the blood sugar of the two groups of patients decreased to a certain extent,and the blood sugar of the sugar-free dialysis group decreased more obviously.Differences between the two groups have statistical significance(P<0.05).3.The incidence of hypoglycemia and asymptomatic hypoglycemia in the sugar-free dialysate group was significantly higher than that in the sugar-containing dialysate group(P<0.05).Conclusion: 1.With the prolongation of a single dialysis time for patients with diabetic nephropathy in the uremic stage,the blood sugar of patients with diabetic nephropathy in the uremic stage shows a downward trend.2.Compared with sugar-free dialysate,patients with uremic diabetic nephropathy who use sugar-containing dialysate for routine hemodialysis have a smaller range of blood glucose fluctuations,and the incidence of hypoglycemia is significantly reduced. |
