Objective:Use cohort research methods to study the clinical efficacy and safety of traditional Chinese medicine decoction combined with paste in the treatment of reflux esophagitis of liver-stomach stagnation-heat,and provide evidence-based medical evidence for clinical application.Methods:1.A total of 51 patients who met the diagnostic criteria for reflux esophagitis of liver-stomach stagnation-heat in the outpatient and ward of the Department of Spleen and Gastronomy of Suzhou Hospital of Traditional Chinese Medicine from January 2019 to December 2020 were included.The treatment group and the control group were respectively 30 cases and 21 cases.2.The treatment group was given oral Chinese medicine decoction+paste+rabeprazole,and the traditional Chinese medicine prescription,namely Zuojin Pill and Jupi Zhuru Decoction,was added or subtracted according to the actual situation of the patient,and taken once every 1 hour in the morning and evening,warmed.The method of taking the paste is to use 3g each of ginseng panax notoginseng powder and bletilla striata powder and a small amount of sugar-free lotus root powder before going to bed and mix thoroughly,boil in hot water,and keep lying down after warming.PPI preparations are used in descending steps according to the differences in endoscopic grading.The control group was given rabeprazole 10 mg orally once in the morning and evening for 8 weeks,and the following 4 weeks were taken as needed,10 mg each time.The two groups of patients were treated for 12 weeks,with follow-up visits and records every 4 weeks,and gastroscopy,electrocardiogram,blood,urine,and fecal routine examinations were performed before and after treatment.After the end of the treatment,the gastroscopy grading,symptom improvement score,RDQ scale,anxiety and depression scale,etc.were compared between the two groups before and after treatment.Six months after the treatment,a follow-up was conducted according to the main symptoms of the RDQ scale.Results:1.General data analysisThere was no statistical significance in the comparison results of basic information,such as age,gender and body mass index,between the two groups before treatment,and the data of the two groups were comparable.2.Analysis of the results of gastroscopyThere was no significant difference between the treatment group and the control group in improving endoscopic esophageal mucosal inflammation(P>0.05),and the curative effect was similar.3.Main symptoms(RDQ scale)curative effect analysisComparison of the total RDQ between the two groups before and after treatment,P<0.01,indicating that the intervention measures of the two groups are effective;the total score of RDQ at 8 weeks and 12 weeks after treatment in the treatment group is better than the control group(P<0.05),and the treatment group improves The main symptoms are better than the control group.The total score of RDQ decreased most significantly in the two groups of patients 4 weeks after treatment.In terms of the efficacy of single symptom,the treatment group was better than the control group in improving heartburn and acid reflux symptoms(P<0.05);and the repeated measures analysis of variance results of acid reflux symptoms(between group F=5.625,P=0.022)suggested that the treatment group It has advantages in improving acid reflux symptoms.The two groups have an advantage in improving chest pain and regurgitation symptoms,P>0.05,there is no statistical difference,and the two groups have similar curative effects.4.Analysis of TCM Syndrome ResultsAfter treatment,the total score of TCM symptoms of the two groups of patients was lower than that before treatment(P<0.01);the treatment group was significantly better than the control group in improving TCM symptoms at 8 and 12 weeks after treatment(P<0.01).The TCM symptom scores of the two groups of patients improved most significantly at 4 weeks after treatment,which was the same trend as the total RDQ score.In terms of the efficacy of syndromes,the total effective rates of TCM syndromes in the treatment group and control group were 93.33%and 76.19%respectively(P<0.05),indicating that the efficacy of TCM syndromes in the treatment group is better than the control group.5.Analysis of Results of Anxiety and Depression ScaleBefore treatment,the two groups of patients were comparable in anxiety and depression scale scores(P>0.05);after treatment,there was a difference in the improvement of anxiety between the two groups(P<0.05),and the treatment group was better than the control in improving anxiety Group:After treatment,there was no significant difference between the two groups in improving depression(P>0.05).6.Analysis of long-term efficacy resultsThe two groups of patients had different degrees of symptom changes during the half-year follow-up after treatment.The total RDQ score of the control group rose significantly higher than that of the treatment group,indicating to a certain extent that the treatment group was better than the control group in improving the long-term efficacy of RE symptoms.7.Safety evaluationThe two groups of patients had no obvious adverse reactions during the treatment,indicating that the medication safety of the treatment group and the control group was good.Conclusion:Compared with the simple application of western medicine,Zuojin Pills and Jupi Zhuru Decoction combined with paste has obvious advantages in treating reflux esophagitis of liver-stomach stagnation-heat,which can relieve clinical symptoms faster,reduce long-term recurrence rate,and reduce anxiety.Comprehensive treatment The effect is good and it is worthy of clinical promotion and application.
