| Objective:To compare the efficacy of foam dressing combined with platelet-rich plasma(PRP)and that of foam dressing alone in the treatment of refractory wounds,in order to provide a reference for the treatment regimen selection of refractory wounds.Methods:154 cases with refractory wound who were not surgically treated from January2017 to August 2020 in Taizhou People’s Hospital were collected.Depending on the treatment regimen,the patients were divided into foam dressing and PRP group(58cases)and foam dressing group(96 cases).In view of the wound healing time/rate,secretion cultivation situation and wound healing quality,the therapeutic effect of the two groups were compared.Results:(1)In the foam dressing and PRP group,7 cases were cured,44 cases were significantly effective,7 cases were effective and 0 cases were ineffective.In the foam dressing group,5 cases were cured,61 cases were significantly effective,27 cases were effective,and 3 cases were ineffective.There was a statistical difference between the two groups(P=0.029<0.05),that is,the wound healing status of foam dressing and PRP group was better than that of foam dressing group.(2)Wound healing time was 18.45±4.74 days on average in the foam dressing and PRP group.And the average wound healing time of the foam dressing group was21.44±5.67 days.The difference in wound healing time between the two groups was statistically significant(P=0.003<0.05),that is,the wound healing time of foam dressing and PRP group was less than that of foam dressing group.(3)The mean wound healing rate of foam dressing and PRP group was90.27±8.11%.And the average wound healing rate of foam dressing group was83.76±c15.54%.There was significant difference in wound healing rate between the two groups(P=0.004<0.05),that is,the wound healing rate of foam dressing and PRP group was higher than that of foam dressing group.(4)In the foam dressing and PRP group,16 cases were positive in secretion culture,and the positive rate of bacterial culture was 27.59%.Besides,43 cases were positive secretion culture in the foam dressing group,and the positive rate of bacterial culture was 44.79%.There was statistically significant difference between the two groups(P=0.033<0.05),that is,the positive rate of bacterial culture in foam dressing and PRP group was significantly lower than that in foam dressing group.(5)In the foam dressing and PRP group,7 cases were mildly abnormal(which had the situation like pigmentation,furfuration and so on),and 0 cases were seriously abnormal(which had the situation like pruritus,ulceration,scar hypertrophy and so on).In the foam dressing group,9 cases were mildly abnormal and 7 cases were severely abnormal.According to the chi-square test,the wound healing quality of foam dressing and PRP group was higher than that of foam dressing group(P=0.036<0.05).Conclusion:Compared with the foam dressing group,foam dressing combined with PRP has a better effect in the treatment of refractory wounds.In addition,PRP can not only shorten the wound healing time and improve the wound healing rate and quality,but also have a certain anti-infection effect.So PRP is worthy of large-scale promotion and use. |
