In recent years,with the wide application of hormone replacement therapy,more and more attention has been paid to the effect of hormone replacement therapy on cardiovascular and cerebrovascular diseases in postmenopausal women.Studies have confirmed that hormone replacement therapy can regulate the level of blood lipids,improve the blood lipid profile,and then delay the occurrence of atherosclerosis.Oxidized low density lipoprotein is an important index to evaluate the risk of atherosclerosis.At present,there are few studies on the effect of hormone replacement therapy on oxidized low density lipoprotein in postmenopausal women.Abstract:To explore the preventive effect of hormone replacement therapy on cardiovascular and cerebrovascular diseases in postmenopausal women by detecting the dynamic changes of oxidized low density lipoprotein and carotid intima-media thickness during hormone replacement therapy in postmenopausal women.To provide new ideas for strengthening the management of blood lipids in postmenopausal women.Methods:A total of 90 postmenopausal women from December 2017 to February 2019 were selected for health examination in the outpatient clinic of the first affiliated Hospital of Hebei North University.The patients were divided into three groups according to their personal wishes and treatment methods: experimental group A(n = 30),experimental group B(n = 30)and control group(n = 30),and followed up for 1 year.The changes ofblood lipid indexes(HDL,LDL,TG,TC,OX-LDL)and modified Kupperman score were recorded before and 1 month,6 months and 12 months after treatment in the three groups.At the same time,the changes of carotid intima-media thickness were recorded before and 12 months after treatment,and the adverse reactions were monitored.Results:1.There was no significant difference in age,menopausal years and BMI among the three groups(P>0.05).There was no significant difference in the observed indexes(HDL,LDL,TG,TC,OX-LDL,MKMI and carotid IMT)among the three groups(P>0.05).2.Comparison of TC,TG,HDL and LDL levels in three groupsOne month after treatment,the levels of TC,TG,LDL and HDL in experimental group A and B had no significant change compared with those before treatment(P>0.05),and there was no significant difference among the three groups(P>0.05).After 6 months of treatment,the levels of TG and LDL in group A decreased(P<0.05),the level of HDL increased(P<0.05),and the level of TC did not change significantly(P>0.05);While in group B,the level of LDL decreased(P<0.05),the level of HDL increased(P<0.05),and the levels of TC and TG had no significant change(P>0.05).The levels of TG and LDL in group A were lower than those in the control group(P<0.05),while the level of HDL was higher than that in the control group(P<0.05),but there was no significant difference in the level of TC(P>0.05).The level of LDL in group B was lower than that in the control group(P<0.05),while the level of HDL was higher than that in the control group(P<0.05),but there was no significant difference in the levels of TC and TG(P>0.05).The level of TG in group A was lower than that in group B(P<0.05),but there was no significant difference in other indexes(P>0.05).After 12 months of treatment,the levels of TG and LDL in group Adecreased(P<0.05),the level of HDL increased(P<0.05),and the level of TC did not change significantly(P>0.05);While in group B,the level of LDL decreased(P<0.05),the level of HDL increased(P<0.05),and the levels of TC and TG had no significant change(P>0.05).The levels of TG and LDL in group A were lower than those in the control group(P<0.05),while the level of HDL was higher than that in the control group(P<0.05),but there was no significant difference in the level of TC(P>0.05).The level of LDL in group B was lower than that in the control group(P<0.05),while the level of HDL was higher than that in the control group(P<0.05),but there was no significant difference in the levels of TC and TG(P>0.05).The level of TG in group A was lower than that in group B(P<0.05),but there was no significant difference in other indexes(P>0.05).3.Comparison of OX-LDL levels among three groupsAfter 1 month,6 months and 12 months of treatment,the level of OX-LDL in experimental group A was lower than that before treatment(P<0.05);While the level of OX-LDL in experimental group B had no significant change compared with that before treatment(P>0.05).At each time point,the level of OX-LDL in experimental group A was lower than that in experimental group B and control group(P<0.05),but there was no significant difference between experimental group B and control group(P>0.05).4.Comparison of intima-media thickness among three groupsAfter 12 months of treatment,the IMT of left common carotid artery,right common carotid artery,left internal carotid artery and right internal carotid artery in experimental group A was slightly lower than that before treatment,and the difference was not statistically significant(P>0.05);While the IMT of left common carotid artery,right common carotid artery,left internal carotid artery and right internal carotid artery in experimental group B was slightly higher than that before treatment,and the difference was not statistically significant(P>0.05).Except for the right internalcarotid artery,the IMT of carotid artery in experimental group A was lower than that in experimental group B and control group(P<0.05).There was no significant difference in carotid IMT between experimental group B and control group(P>0.05).5.Comparison of the improvement of peri-menopausal symptoms among the three groupsAfter 1 month,6 months and 12 months of treatment,the modified Kupperman scores of experimental group A and experimental group B were lower than those before treatment(P<0.05).At each time point,the modified Kupperman scores of experimental group A and experimental group B were lower than those of the control group(P<0.05),but there was no significant difference between experimental group A and experimental group B(P>0.05).Conclusion:1.Hormone replacement therapy can reduce LDL and TG,and increase HDL in postmenopausal women,thus improving blood lipid levels.2.Hormone replacement therapy can reduce the level of OX-LDL in postmenopausal women,delay the thickening of intima-media thickness of carotid artery,and reduce the risk of cardiovascular and cerebrovascular diseases.3.Hormone replacement therapy can decrease the modified Kupperman score of postmenopausal women and improve the symptoms related to peri-menopausal syndrome.
