| Objective: to verify the feasibility of the self-developed new type of temporary intestinal deposit device in beagle dogs with single intestinal gunshot injury model.Methods: five female beagles were anesthetized 12 hours after fasting.A 10 cm incision in the middle of abdomen was taken,and a incision protector was placed.The intestine was exposed naturally,and a small intestine bullet wound model was made by shooting the intestine tube with an automatic rifle 25 m away.The ruptured intestine was reconstructed by a new type of self-developed temporary intestinal deposit device,and the suspected necrotic intestine was deposited,then the abdominal cavity was temporarily closed,and the laparotomy was performed 4hours later.To observe the simple treatment time,transit time,emergency treatment time and treatment time of each section of intestine in the fire line stage,the vitality of the intestine after 4 hours,take the heart,liver,spleen,lung,kidney,stomach,normal active intestine before and after the operation,suspicious necrotic intestine for light microscopic pathological examination,take the broken end of the intestine connected with the intestinal reconstruction device before and after the operation for transmission electron microscopic pathological examination.Results: the simple treatment time in the fire line stage was 41.55 ± 10.46 s,which was lower than the maximum time limit in the field emergency principle.The transit time was 60.78 ± 15.95 s,which was lower than the maximum time limit of this stage in the principle of first aid on the battlefield.The emergency treatment time was 29.75 ± 5.13 min,and the reconstruction time of each segment of intestine was 4.44 ± 0.31 min.One Beagle died of spleen rupture and bleeding,and the other four dogs were all evacuated.Four hours after the emergency operation,the abdominal cavity was explored,and the intestinal deposit device was fixed in place.The activity of the reconstructed intestinal tube was normal without obvious necrosis.The suspicious necrotic intestinal tube had the same activity as that before the deposit.Light microscopy and pathological examination showed that there was no obvious necrosis in heart,liver,spleen,lung,kidney and stomach tissue sections,no obvious inflammatory cell infiltration,no obvious necrosis in normal intestine before and after operation,a small amount of inflammatory cell infiltration,villi and local necrosis in tissue and a large number of inflammatory cells infiltration in suspected necrotic intestine before and after operation.The pathological examination of transmission electron microscope showed that the villi at the broken end of the intestine connected with the device before operation were intact without obvious necrosis,and the villi at the broken end of the intestine connected with the device after operation were moderately damaged with focal necrosis.Conclusion: 1.The model of small intestine bullet injury in beagle dogs was established successfully.2.It is feasible and effective to apply the principle of DCS combined with the principle of first aid in the field of war to the model of small intestine bullet injury in beagle dogs.3.The self-developed new temporary intestinal deposit device can be used in the emergency treatment of beagle dogs with simple small intestinal gunshot injury,and can avoid further deterioration of intestinal tissue. |
