Study On Risk Factors Of Acute Acalculous Cholecystitis For Patients With Multiple Trauma

Abstract/Summary:
Objectives This study aims to take an investigation on the incidence and risk factors of acute acalculous cholecystitis(AAC)for patients with multiple trauma.From this study,more intuitive data can be collected to catch more attention to the AAC from the clinical doctors.The patients facing the risk of AAC should be reexamined as regularly as possible in order to find the factor and take the according treatment in time,which will reduce the impact of AAC on patients with multiple trauma,shorten the hospital stays,and contribute to early rehabilitation and discharge.Methods The clinical data of patients with multiple trauma from the traumatology department in a large hospital in Wuhan from January 2019 to December 2019 were retrospectively analyzed and divided into two groups,namely the AAC group and the non-AAC group.The information of all the patients meeting the requirement was recorded,including the patient’s age,gender,history of smoking and drinking,history of underlying diseases,injury mechanism,Glasgow coma scale(GCS score),injury severity score(ISS score),acute physiology and chronic health evaluation(APACHEII score).The occurrence of abdominal injury,emergency surgery,hemorrhagic shock,blood transfusion therapy and mechanical ventilation was also put on record together with the patient’s fasting time during hospitalization,mean arterial pressure measured on admission and white blood cell count measured on admission.At the beginning,single factor analysis method was applied on all the possible risk factors researched and the difference of various indicators was compared between the AAC group and non-AAC group to testify the existence of statistical significance.Then the influence factors with significant difference was imported to the bivariate regression model of Logistics,and the related risk factors of AAC were attained,together with the corresponding OR value and 95% CI.Results According to the data,the incidence of AAC for patients with multiple injuries was calculated to 8.64%.In comparison of each possible influencing factor between the patients in AAC group and those in the non-AAC group,the indicators of the AAC group such as the ISS rating,APACHEII scores,the occurrence of abdominal injury,the application of mechanical ventilation,the existence of hemorrhagic shock,the employment of blood transfusion treatment and fasting time all differ significantly from those of the latter group,which is statistically significant(P < 0.05).When the seven risk factors were imported to the bivariate regression model of Logistics,it turned out that the existence of hemorrhagic shock(OR = 0.034,95% CI 0.004-0.261,P=0.001)is an independent risk factor for the occurrence of AAC.Conclusions The incidence of AAC for patients with multiple trauma is8.64%,and hemorrhagic shock forms an independent risk factor for its occurrence.For patients with hemorrhagic shock,more attention should be paid to the occurrence of AAC.Regular review,early discovery and timely treatment should be exercised to reduce the impact of AAC on the treatment of patients with multiple injuries,which is also conducive to the early rehabilitation of them.
