The multi-parameter bedside monitor can monitor the changes of patients’ vital signs and provide objective and effective real basis for clinical diagnosis.Therefore,it is widely used as the basic equipment configuration in hospitals at all levels.However,for patients admitted to the general ward of the hospital,the bedside monitor cannot realize remote centralized monitoring,and the medical personnel can only enter the ward where the patients are and manually record the physiological parameters of the patients.At the same time,the alarm threshold of the monitor is generally set low.Too many errors and meaningless alarms make the medical staff run back and forth between different wards,which makes them tired physically and mentally.And multi-parameter monitor dynamic ECG monitoring as an effective method for the screening of asymptomatic patients with atrial fibrillation,but will be read by the attending physician is stored at the large number of segments of electrocardiogram(ECG)monitor,according to the form and rhythm characteristics of ECG signal to carry on the diagnosis of atrial fibrillation,big workload and diagnosis is closely related to the clinical experience.In view of this situation,in order to reduce the cost of medical monitoring,relieve the pressure of medical resources in China,and reduce the workload of medical staff,this paper designed and developed a mobile monitoring system for inpatient medical care applied in the general ward of the hospital.The system is divided into two parts.One part is a background monitoring system based on B/S mode,which realizes remote real-time monitoring of patients’ physiological signals by using dynamic web pages.In addition,atrial fibrillation detection algorithm was implanted to efficiently screen out suspected fragments of atrial fibrillation,reduce the amount of doctors reading ECG fragments,and improve the diagnostic efficiency and accuracy of atrial fibrillation.The other part is the mobile monitoring information APP for inpatient medical care based on the Android platform,which is suitable for mobile intelligent devices to make up for the inconvenience of carrying on the PC terminal.The two monitoring modes have complementary functions and are suitable for different usage scenarios.The medical staff can realize the full time uninterrupted care for the vital sign parameters of patients in multiple rooms and different beds through the mobile APP or the PC website,receive and selectively process the alarm information of the monitor,and make the correct judgment on the patient’s condition in time.The main research contents of this paper are as follows:(1)Study and analyze the monitoring signal acquisition and alarm principle,combined with the current working status of the general care ward of hospitals at all levels,conduct demand analysis on the system function,and complete the design of the overall functional structure of the system.(2)Design atrial fibrillation detection algorithm based on RR interval feature recognition,which mainly includes two aspects,namely,the determination and screening of atrial fibrillation fragments by adopting adaptive threshold differential algorithm to location R wave and ECG signal respectively.And transform it into JAVA language,implant background monitoring system(3)Using componentized programming idea,combining HTML and CSS technology to design Web front-end interactive pages,and build Web application server and database server.MVC architecture was adopted to develop the mode organization program code,and the functions of user login,basic patient information display,dynamic and real-time monitoring of physiological parameters,alarm information display,detection and identification of atrial fibrillation signals,abnormal waveform fragments and automatic data storage were realized in the remote monitoring system based on B/S mode.(4)Based on the need for medical staff to know the vital signs of patients being monitored at any time,a mobile monitoring information APP for inpatient care was designed to realize real-time query of patient monitoring data,push and save alarm information on mobile terminal devices.The research and development of this system establishes a new medical mode between the medical staff and the patients under care in the general ward,and provides a new method for improving the quality of medical care,reducing the complexity of labor and improving the work efficiency of medical workers.
