| This paper identifies and analyzes the most frequent hazardous behaviors and causal factors found within Part 121 and Part 135 accidents and incidents. The research investigative process consisted of a literature review and statistical data analysis of accident reports received from the National Safety Transportation Board (NTSB) and incident reports received from NASA's Aviation Safety Reporting System (ASRS) program.;The NTSB reports ranged from January 1983 to December 2000. The ASRS incident reports ranged from January 1990 to December 2001. The NTSB provided 414 accident reports initially. The ASRS provided 360 reports. The reports were then reviewed for completeness and content. Several reports were purged because they were either incomplete, did not match the criteria for this paper, or were duplications. Statistical analysis of 303 accidents and 33 incidents were then conducted using SPSS 10.1(TM) statistical software. Frequency distributions, regression, and correlation analysis were conducted to identify HB/CF frequency distributions and relationships between behavioral and demographic data.
