A Study On The Social Work Discharge Plan Of Children With Kidney Disease

Discharge planning for medical social work is the continuation of continuous treatment and nursing.Throughout the period of hospitalization,the discharge planning mode has undergone changes from social work service mode to nursing service mode to medical integration team service mode.Based on the practice research of the medical social worker in S ward of Children’s Hospital of X City and using the theory of cognitive behavior and social support network theory,this paper mainly uses case social work method,takes 5 children with renal diseases as service targets.This paper analyzes the application of the social work discharge planning in the care of children with kidney disease.The study finds that the social work-oriented discharge planning model led by medical social workers is mainly applied to the adaptation of children with kidney diseases to the environment,the coordinated use of resources,the protection of continuous treatment,and the maintenance and promotion of daily life functions.By working with health care workers,children with nephrology and parents,the medical social worker will comprehensively assess the needs of children with kidney disease,change their unreasonable beliefs,expand their support networks and tap the potential of children.Together,they will work out a targeted discharge plan.The medical social worker mainly plays the role of assessor,liaison and supporter.The results showed that the formulation of social work discharge planning not only had a positive impact on the physical,psychological and social behaviors of 5 children with nephrotic syndrome but also reduced the cost of medical services in S district by reducing the length of hospital stay and the rate of rehospitalization to a certain extent.Making the effective utilization of medical resources in S ward and alleviating the current situation of tight beds in S ward.Through exploratory research on 5 nephrotic children in S ward,medical social workers established the application mode of discharge plan in children with nephropathy and provided reference for the research on discharge plan in China.
