ObjectiveThis study was to analyze and evaluate the related factors of childhood asthma in Shanghai through questionnaires,which may provide reference for the development of clinical prevention strategies in the future;and to summarize the timing of children's asthma attacks and aggravation,which may provide theoretical basis for“time-intervention”and “time-time treatment”.MethodsThe children with asthma in the outpatient department of Shanghai Traditional Chinese Medicine Hospital were treated as the case group.The healthy children in primary school and kindergarten were used as the control group at the same time.The questionnaire on children's asthma related factors was drawn up by rhe two groups.We followed up regularly and recorded the duration of asthma attacks and aggravation.Univariate analysis and logistic regression analysis were used to analyze the results of related factors.The statistical method of round distribution was used to analyze the time of asthma attack,and the risk factors and time course of asthma in Shanghai children were summarized.Results(1)Investigation results of asthma related factors in Shanghai children: 540 cases of research samples were collected in this study,174 cases(64.4%)of male children in the case group,96 cases(35.6%)of female children,age 6.49±2.49,control group There were 165 male children(64.4%)and 105 female children(38.9%),age of 6.16±2.69.There was no significant difference in age-specific ratio between the two groups(P<0.05).In the growth and development,the body weight and BMI index of the case groups were lower than that of the control group(P<0.05).There was no significant difference in nationality and height(P>0.05).In the case of birth,the production patterns of the two groups were statistically different(P<0.05),and the proportion of caesarean section in the case group was higher(54.1%).There was no significant difference in pregnancy,birth weight and feeding pattern between the two groups(P>0.05).In the personal and family allergy history,the proportion of allergic diseases,food allergies,odor allergies,mites allergies,pollen allergy,family asthma,and other family allergic diseases in the case group was significantly higher than that in the control group,and the difference was statistically significant(P< 0.05),there was no significant difference in other diseases and drug allergy between the two groups(P>0.05).In theresults of the above analysis,eczema(n=110,53.1%)and allergic rhinitis(n=104,50.2%)accounted for a high proportion of personal allergic diseases;eggs(n=39,42.4%)Milk(n=31,33.7%),seafood(n=24,26.1%)are more common in food allergy history;family history is mostly parents(n=60,69.7%),grandparents(n=12,14.0%)suffering from asthma;family allergic diseases are more common with allergic rhinitis(n = 64,61.0%)and eczema(n = 46,43.8%).In the indoor environment,the proportion of the asthma group with decoration/new furniture,playing with fluffy toys,smoking at home,and obvious pollution around the house was significantly higher than that in the healthy group,the difference was statistically significant(P<0.05).The proportion of asthmatic patients who were exposed to radiation products and animals at home was significantly lower than that of healthy ones,and the difference was statistically significant(P<0.05).There was no significant difference between the two groups in the interior of the carpet,the cotton used in the bedding material,and the flowers in the house(P>0.05).Logistic stepwise regression analysis showed that allergic diseases(OR=19.452),mites allergies(OR=2.267),family history of asthma(OR=4.641),decoration/new furniture within half a year(OR=14.820),liking to play with fluffy toys(OR=2.733),smoking at home(OR=4.249),which had high more possiblility to expose in asthma.And planting flowersat home(OR=0.473)was a protective factor against asthma.(2)During January to December in 2018,a total of 55 patients were followed up about the time of children's asthma attacks and aggravation.The most frequently occurring months were concentrated in March,April,May in Spring,and June,July,August,in summe.September,October,and November were more evenly distributed,which is autumn,and less incidence in December,January,and February.The daily attacks on asthma focused on between the time from 22:00 to 8:59,that was54 times(65.85%).The most frequent occurrence of asthma attacks was Yin Shi(3:00a.m?4:59 a.m)and Chen Shi(7:00 a.m?8:59 a.m).The number of asthma attacks was less from 9:00 to 21:59 during the day,that was 28 times(34.15%).The results of circular distribution statistical analysis showed that there was no trend of the number of monthly seizures in 55 children with asthma(P>0.05),and the number of daily episodes was concentrated(P<0.05),the average angle has statistically significant,the corresponding time was 3:15 a.m,and the angle difference was 0.6038.Conclusion(1)Caesarean section,allergic diseases,food allergies,odor allergies,mites allergies,pollen allergy,family asthma history,decoration / new furniture within six months,liking to play with fluffy toys,smoking at home,obvious pollution around the residence,which related to the onset of asthma.And the risk factors for asthma were allergic diseases,mites allergies,family history of asthma,decoration/new furniture within half a year,and the use of fluffy toys.(2)Eczema and allergic rhinitis were common in personal and family allergic diseases.Food allergies were more common with eggs,milk and seafood proteins.In families asthma history parents had asthma.(3)The monthly onset of asthma was concentrated in March?August.It was a spring and summer season for the asthma season.(4)The daily onset of asthma was mostly concentrated from 22:00 to 8:59,and less from9:00 to 21:59.The most frequent occurrence of asthma attacks was Yin Shi(3:00 a.m?4:59 a.m)and Chen Shi(7:00 a.m?8:59 a.m),and the peak of the attack was 3:15 a.m.ObjectiveThe purpose of this study was to observe the clinical effect of Huayu Pingchuan Recipe in the treatment of acute asthma attack in children.MethodsChildren with asthma who met the inclusion criteria were divided into treatment group and control group by digital randomization method.On the basic treatment of Aerosol Budesonide and Terbutaline Suspension,Huayu Pingchuan Recipe and Maxingshigan Decoration were given respectively.After 1 week of treatment,the TCM syndrome scores,blood granulocyte percentage and platelet count changes before and after treatment were observed to evaluate the clinical efficacy.Results(1)78 children with asthma met the criteria.3 patients lost in the treatment group,and 3lost patients in the control group.The total shedding rate was 7.6%.There were 36 in the treatment group,and 36 in the control group.The differences between the two groups in age,sex had no statistical significance(P>0.05),so the two groups of children was comparable.(2)Compared with before the treatment,the TCM syndrome of cough,chest tightness,wheezing,snoring and main symptom scores of the two group were sign-ificantly reduced(P<0.05).Compared with the control group,the TCM syndrome of cough,chest tightness,wheezing,snoring and main symptom scores of the treatment group were significantly reduced,(P<0.05).(3)Compared with before the treatment,the TCM syndromes of fever,face redness,phlegm thick and secondary syndrome of two groups were all decreased,and the difference was statistically significant(P<0.05).Compared with the control group,the TCM syndromes of face redness,phlegm thick,and secondary syndrome of the treatment group were significantly decreased.(P<0.05).There were no significant differences in the scores of fever(P>0.05).(4)Compared with the groups before treatment,the eosinophil percentage,platelet counts in the two groups were decreased after the treatment,and has statistically significant difference(P<0.05).Compared with the control group,the eosinophil percentage and platelet count in the treatment group were more significant declines(P<0.05).(5)The total effective rate of the treatment group(97.4%)was better than the total effective rate of the control group(86.2%),and the difference was statistically significant(P<0.05).ConclusionHuayu Pingchuan Recipe could improve symptoms such as wheezing,cough,chest tightness and snoring,and reduce the eosinophi ls percentage and platelet count.Huayu Pingchuan Recipe had good clinical efficacy in the treatment of acute asthmatic attack in children.
