Objective:In this study,a randomized controlled clinical study was conducted to observe the frequency of onset,duration of disease,total score of TCM syndromes and syndrome points of TCM before and after intervention,and to explore the effectiveness and clinical application of the three-step Tuina method in the treatment of children with recurrent respiratory tract infection after a certain course of treatment with the three-step Tuina method and the traditional manipulation.Methods:This study followed the five basic principles of randomized control,clinical trial ethics,and informed consent.Children aged 2-5 years old with dual deficiency of the lung-spleen Qi who met the criteria for diagnosis and inclusion of repeated respiratory tract infection were selected from the Tuina Out-patient department of Yueyang Hospital affiliated to Shanghai University of Traditional Chinese Medicine from April25,2018 to August 19,2018.100 children who met the criteria for diagnosis and inclusion were selected as subjects.The three-step Tuina group was treated with "pressing and rubbing acupoints,abdomen and chiropractic",while the traditional manipulation group was treated with the method of treating repeated colds in children according to the Ministry of Health’s "Twelfth Five-Year Plan" textbook "Children’s Tuina".The two groups were treated 3 times a week for a total of 36 times for 3months.Statistical observations were made at the end of treatment and 3 months after the end of treatment.Statistical software was used to objectively analyze and evaluate the immediate efficacy at the end of treatment and the short-term efficacy at follow-up.Results:A total of 100 children were included in the study,1 cases were actively withdrawn in the three-step Tuina group,and 49 cases were finally completed.There were 2 cases of other diseases in the traditional manipulation group,and parents demanded withdrawal and finally completed 48 cases.There was no statistically significant and comparable general data groups in the first two groups of children,such as age,sex,height,weight,number of cases,disease course and total score of TCM syndromes.(1)Comparison of clinical effective rates between two groups of children:At the end of treatment,the total effective rate of the three-step Tuina group was 93.9%,the traditional manipulation group was 89.6%,and there was no significant difference in efficiency between the two groups.However,the efficiency of the three-step Tuina group was 46.9%,the traditional manipulation group was 35.4%,and the efficiency of the three-step Tuina group was significantly higher than that of the traditional manipulation group.At the time of follow-up,the total effective rate of the three-step Tuina group was 87.8 %,and the traditional manipulation group was 62.5 %.It is suggested that the three-step Tuina method and the traditional manipulation can effectively improve the recurrent respiratory tract infection in children,and have better short-term curative effect,and the three-step Tuina method is superior to the traditional manipulation,especially in the short-term curative effect.(2)Comparison of the number of children suffering from lower respiratory tract infection between the two groups: After treatment,the number of children suffering from lower respiratory tract infection in the three-step Tuina group and the traditional manipulation group has been significantly improved,with statistical differences before and after treatment and during follow-up and before treatment(P<0.05).But no matter at the end of treatment or at the time of follow-up,there was no statistical difference between the two groups(P>0.05).It is suggested that three-step Tuina and traditional manipulation can significantly improve the frequency of children suffering from lower respiratory tract infection and have better short-term curative effect.The immediate curative effect of the two methods is equivalent to the short-term curative effect.(3)Comparison of the disease course between the two groups: After treatment,the disease course of the children in the three-step Tuina group and the traditional manipulation group has been significantly improved,and there is a statistical difference between before and after treatment,during follow-up and before treatment(P<0.05).At the end of treatment,there was no statistical difference between the two groups(P>0.05),but during follow-up,there was a difference between the two groups(P<0.05).It is suggested that both three-step Tuina and traditional manipulation can significantly shorten the disease course of children with recurrent respiratory tract infection and have better short-term curative effect,while the short-term curative effect of three-step Tuina is better than traditional manipulation in shortening the disease course.(4)Comparison of TCM syndrome points between two groups of children:At the end of treatment and at the time of follow-up,the total points and symptoms of TCM syndromes in the three-step Tuina group and the traditional manipulation group included yellow face,less food,fatigue,sweatiness,and stool irregularities than before treatment,and the difference was statistically significant(P<0.05).The TCM syndrome total score and the nano-drinking score in the three-step Tuina group were significantly lower than the traditional manipulation group at the end of treatment and follow-up.The difference was statistically significant(P<0.05).It is suggested that the three-step Tuina method and the traditional manipulation have significant effects in improving the lung-spleen Qi deficiency in children,and the three-step Tuina method is more effective.(5)Comparison of height and weight between two groups of children:Referring to the physical development measurements(x ±s)of children under 7 years of age in nine major urban areas of China in 2005,a five-grade division method was used to classify the height and weight of the child before and after treatment.After the follow-up with the treatment of the predecessor of high,weight grade ratio comparison,after treatment,the child after treatment can reach the average height and weight of children of the same age group,or even more than the height and weight of children of the same age.There was no statistically significant difference between the groups(P>0.05),suggesting that the three-step Tuina method and traditional manipulation could promote the development of children’s height and weight,and that the two methods had the same curative effect.Conclusion:1.The three-step Tuina method and the traditional manipulation have significant immediate curative effect and short-term curative effect on reducing the number of cases,shortening the course of disease and improving physique,promoting height and weight development in children with recurrent respiratory tract infection of dual deficiency of the lung-spleen Qi deficiency.2.In terms of total efficiency,improving the overall physique of children,and improving the symptoms of children with less food,Compared with traditional techniques,the three-step Tuina method has better immediate curative effect and short-term curative effect.3.In shortening the disease course,the three-step Tuina method has better curativeeffect than the traditional manipulation in the near future.
