Part 1.Analysis of Medical Cases of Postoperative Thyroid CancerOBJECTIVE:Based on the excavation and analysis of the medical records of thyroid cancer after surgery,the clinical characteristics of the patients and the experience of Professor Wei Zixiao in the treatment of thyroid cancer after syndrome differentiation were summarized.METHODS:According to the inclusion and exclusion criteria,complete cases of thyroid cancer after surgery treated by Professor Wei Zixiao from 2013 to 2019 were collected,a total of 84 cases were included,and a total of 163 visits were collected,and general information,medical history,symptoms,tongue,pulse,syndrome,Clinical data such as treatment methods,prescriptions,etc.,and the establishment of an Excel electronic database,statistical analysis of the entered data using SPSS 26.0 and traditional Chinese medicine inheritance support system(V2.5)software,to explore the clinical information distribution of patients,etiology and pathogenesis,and Wei Professor's experience in dialectical medicine after treatment of thyroid cancer.RESULTS:General data statistics show that the 84 patients included in the ratio of males and females is 1:6.64;the average age is 45.05 years,middle-aged group>young group>elderly group;the most patients with a disease course of less than 1 year,and the least patients with a disease course of more than 5 years;Of the patients 94.05%were DTC;98.73%of the patients after DTC were treated with orally and the average dose of orally was 107.32?g/d;72.15%of the patients had tested TSH levels within 3 months,of which 29.11%of the patients TSH<0.1 mU/L.Data mining results of symptoms,tongue signs,pulse signs,syndrome types,and rule of law show that the main pathogenesis of thyroid cancer after surgery is complicated,of which evil qi is more abundant and right qi is weak.The actual evil involves Qi stagnation,sputum coagulation,blood stasis,and fiery fever.Zhengxu involves qi deficiency,blood deficiency,yin deficiency,and yang deficiency,and yin deficiency,blood deficiency,and qi deficiency are common.The data mining results of drug frequency,Siqi,Wuwei and Guijing show that Siqi is mainly cold and warm,and bitterness is the largest proportion in Wuwei,which is mainly attributed to liver meridian and spleen meridian.The efficacy of the drug is centered on clearing heat and detoxifying,clearing away heat and dissolving stagnation,relieving liver and qi,and relieving liver and nourishing blood.Association rules showed that the core drug combination was Bupleurum-Yujin-Hedyotis diffusa.Commonly used drug combinations are:Prunella vulgaris-Hedyotis diffusa,Curcuma diffusa-Hedyotis diffusa,Baishao-Yujin,Bupleurum-Yuj in,Polygala-Shichangpu.Drug clustering yielded 7 new prescriptions.Analysis of the relationship between drugs and syndromes yielded the core drugs of five main syndrome types,including liver-qi stagnation syndrome,stasis and heat injury syndrome,qi-yin deficiency syndrome,spleen and kidney deficiency syndrome,and phlegm-stasis mutual syndrome.CONCLUSION:The data mining results objectively reflect the academic thoughts and clinical experience of Professor Wei Zixiao after treatment of thyroid cancer.The obtained results refined and refined Professor Wei's subjective experience in terms of drug properties and drug combination models.For some potential medication laws obtained through data mining,clinical tests are also required to determine whether they have value.Part 2.Network Pharmacology Study of Core Drug Combinations OBJECTIVE:Using the research methods of network pharmacology,combined with open databases,to explore the main components and mechanism of action of the core drug combination.METHODS:The active ingredients and target targets of the core drug combination were obtained through the Chinese Medicine System Pharmacology Database and Analysis Platform(TCMSP),and the targets were genetically annotated according to the UniProt protein database.Obtain genes related to thyroid cancer by querying the GeneCards database Using R scripts to screen genes common to drugs and diseases,that is,potential targets.The Cytoscape 3.2.1 software was used to build the drug-component-target-disease network.Use the STRING database to construct a map of the interactions between target proteins and find key genes.Using R script to perform gene function enrichment analysis of target genes.Gene pathway analysis was performed using the DAVID Bioinformatics Resources 6.8 database.RESULTS:The active ingredients of the core drug combination Bupleurum-Yuj in-Hedy otis diffusa include 2-methoxy-3-methyl-9,10-anthraquinone,quercetin,isorhamnetin,kaempferol,3,3",4",5,5",6,7-hexamethoxyflavones,acapulin,petunia,naringenin,among which quercetin,isorhamnetin,kaempferol and the target Points are closely related.Potential targets are 23 genes including the estrogen receptor and epidermal growth factor receptor.Among them,MYC proto-oncogene,AKT serine/threonine kinase 1,epidermal growth factor receptor,Erb-B2 receptor tyrosine kinase 2 may be the more critical genes.Gene function analysis showed that target genes were significantly concentrated on receptor activity and transcription factor activity,with a high proportion concentrated on cytokine receptor binding and receptor ligand activity.No pathway for target gene aggregation was found.CONCLUSION:The core drug combination Bupleurum-Yujin-Hedyotis diffusa intervenes in thyroid cancer,which works through multiple components and multiple targets.The main active component is flavonoids.The results of key genes and gene function analysis provide directions for studying specific mechanisms of action.
