| Background and aim:Helicobacter pylori associated gastritis is a common disease in clinical.As a pathogenic factor,helicobacter pylori can cause gastric mucosa atrophy,intestinal metaplasia,even can cause atypical hyperplasia after a long-term persistent infection.Eradication of Hp is the fundamental method for the treatment of helicobacter pylori associated chronic gastritis,various of uncomfortable symptoms of patients can be relieved and the histopathological changes of gastric mucosa can be reversed at a certain extent,so it has been a hot topic in clinical research.However,in the context of antibiotic resistance in China,treatments for the eradication of helicobacter pylori has been constantly updated due to the gradually reduced eradication rate.According to the latest fifth national helicobacter pylori treatment consensus,the quadruple therapy which cosisit of two kinds of antibiotics were mostly used in clinical But many researchs have showed the eradication rate of helicobacter pylori cannot maintain at a satisfactory level which is above 90%.However,with the continuous extension of drug dosage and cycle in the course of treatment,the adverse reactions and the drug resistance of helicobacter pylori has been gradually increased,the bottleneck problem in the process of Hp eradication has becoming increasingly obviously.Due to the unique advantages of syndrome differentiation of traditional Chinese medicine therapy,the usage of TCM in both clinical and basic researchse have shown a good prospect.Traditional Chinese medicine can improve the clinical symptoms of patients with helicobacter pylori associated chronic gastritis,reduce the adverse reactions during the quadruple therapy,and also can improve the eradication rate of Hp.JingHuaWeiKang Caspusle,as a commonly used Chinese patent medicine in clinical practice,has many advantages such as good treatment effect for a variety of digestive system diseases,easy to take,and has a high acceptance of patients.Therefore,this paper intended to evaluate the therapeutic effect of JingHuaWeiKang Caspusle combined with quadruple therapy on chronic gastritis patients with helicobacter pylori infection,and explore the advantage parts of JingHuaWeiKang Caspusle in the treatment of patients,in order to provide a reference for the clinical selection of more targeted drugs.Methods:This study was a practical randomized controlled clinical research,according to the selection standard,a total of 77 patients with helicobacter pylori associated chronic gastritis and were identified as spleen and stomach Dampness-heat syndrome who were admitted to the outpatient department of digestive center in Beijing hospital of traditional Chinese medicine affiliated to capital medical university from May 2019 to January 2020 were selected.The patients were randomly divided into control group(38 cases)and treatment group(39 cases)through the method of random number generation software.The control group was given omeprazole sodium enteric-coated tablets+bismuth potassium citrate capsules+amoxicillin capsules+clarithromycin tablets for 14 days,and the treatment group was given JingHuaWeiKang Capsule combined with quadruplex therapy for 14 days.At the end of treatment,the eradication rate of helicobacter pylori,the remission degree of clinical symptoms and the occurrence of adverse reactions of 70 patients in the two groups were evaluated by 13C breath test and TCM syndrome evaluation scale.The degree of remission of clinical symptoms included TCM syndrome score and single symptom score.Results:In this study,a total of 70 patients were enrolled,35 patients in the control group and 35 patients in the treatment group.1.Analysis of the chief complaint:Among the 70 patients with helicobacter pylori associated chronic gastritis with Dampness-heat syndrome of spleen and stomach,upper abdominal distension,upper abdominal pain and halitosis were the most high-frequency symptoms.2.Helicobacter.pylori eradication rate:After ITT analysis and PP analysis,there were no significant difference in helicobacter pylori eradication rate between the control group and the treatment group.The eradication rate of helicobacter pylori in the treatment group was higher than that in the control group among 40 patients with spleen and stomach Dampness-heat syndrome with helicobacter pylori associated chronic gastritis in the Pi-Man group.3.Syndrome efficacy:The syndrome score of the treatment group after treatment was significantly lower than the score before the treatment(P<0.05).After the treatment,the syndrome score of the treatment group was significantly lower than that of the control group(P<0.05).After the treatment,the total effective rate of the treatment group was significantly higher than control group(P<0.05),the clinical recovery rate,apparent efficiency and effective rate were all higher than the control group.4.Single symptom efficacy:After the treatment,the score of upper abdominal distension and halitosis in the treatment group was significantly lower than the control group(P<0.05).After the treatment,the total effective rate of upper abdominal distension,poor appetite and bitter taste in the treatment group was significantly higher than the control group(P<0.05).5.Incidence of adverse reactions:There were no significant differences between the control group and the treatment group in the incidence of adverse reactions.Conclusion:According to the results of this study,the addition of JingHuaWeiKang Caspusle in the routine quadruple therapy had a certain synergy.It could increase the rate of helicobacter pylori eradication in some way,and could also significantly improve the patients’ clinical symptoms,especially in improving the upper abdominal distention,poor appetite,bitter taste,and halitosis symptoms.It was suggested that in the clinical treatment of helicobacter pylori associated chronic gastritis with spleen and stomach Dampness-heat syndrome,traditional Chinese medicine diagnosis of Pi-Man patients,especially have symptoms such as upper abdominal distension,poor appetite,bitter taste and halitosis,in the premise of considering the overall situation of the patients,the doctor can choose to supplement with JingHuaWeiKang Caspusle in order to improve the clinical efficacy. |
