Objectives:The objectives of this study are to evaluate the clinical effectiveness and the influence on patients’ illness severity of Qipi San combined with routine treatment in ICU diarrhea patients with cold-heat complex syndrome,via comparing the changes of diarrhea scores,TCM symptom and syndrome scores,APACHE II scores,leucocyte count,neutrophil percentage and CRP before and after the corresponding treatment,which could provide innovative methods for ICU diarrhea patients in the future.Methods:This study used a retrospective cohort study method,by collecting all the inpatients’ data(from January 1st,2016 to December 31st,2019)from the Hospital Information System of Guang’anmen Hospital affiliated China Academy of Chinese Medical Sciences.According to the inclusion and exclusion criteria for case selection,60 patients using combined treatment are defined as the Qipi San group,other 51 cases only using routine treatment are as the control group,both within therapy period of 7 days.Statistical analysis was carried out by SPSS22.0,the counting data were analyzed by chi-square test,and the measurement data were expressed by mean 1standard deviation.When comparing between groups,the measurement data in accordance with the normal distribution were tested by independent sample t-test,and the data that did not accord with the normal distribution were tested by Wilcoxon rank sum test.Paired t-test was used for intra-group comparison.Statistical significance was set at P<0.05.Results:1.Common dataA total of 111 eligible cases were included in this study.60 patients were included into Qipi San group(30 males and 30 females).Age ranges from 39 to 91 with average of 74.3±12.8.Enteral nutrition support in 36 cases;Antibiotics using in 57 cases;Mechanical assisted ventilation in 23 cases;Hypoproteinemia in 48 cases.APACHE Ⅱ average score was 24.48±5.40.51 patients were included into the control group(28 males and 23 females).Age ranges from 54 to 88 with average of 73.4±9.6;Enteral nutrition support in 30 cases;Antibiotics using in 50 cases;Mechanical assisted ventilation in 22 cases;Hypoproteinemia in 40 cases.APACHE Ⅱ average score was 23.37±5.78.There was no statistically significant difference in general data between the two groups(P>0.05).2 Score and efficacy of diarrheaBefore treatment,there was no significant difference in the scores of diarrhea between the two groups(P>0.05);after treatment,the scores of diarrhea between the two groups were significantly lower than before treatment(P<0.01);and after treatment,Qipisan group The score of diarrhea was significantly lower than the conventional treatment group(P<0.05).After treatment,13 cases of diarrhea symptoms were cured in Qipisan group,32 cases were markedly effective,11 cases were effective,and the total effective rate was 93.3%;the conventional treatment group was clinically cured,markedly effective and effective cases were 8 cases,20 cases,14 respectively For example,the total effective rate is 82.4%.The total effective rate of Qipisan group was significantly higher than that of conventional treatment group(P<0.05).3 Changes in the severity of the disease3.1 APACHE Ⅱ scoreThere was no significant difference in APACHE Ⅱ score between the two groups before treatment(P>0.05);after treatment,the APACHE Ⅱ score of the Qipi San group was significantly lower than that before treatment(P<0.01),and there was no statistical change in the APACHE Ⅱ score of the conventional treatment group The academic difference(P>0.05);the APACHE Ⅱ score of the Qipisan group after treatment was significantly lower than that of the conventional treatment group(P<0.05).3.2 Inflammation indexThere was no significant difference in WBC,NEUT%and CRP between the two groups before treatment(P>0.05);after treatment,the WBC,NEUT%and CRP of the two groups were significantly lower than before treatment(P<0.05),but two after treatment There was no significant difference in WBC,NEUT%and CRP between groups(P>0.05).Conclusions:1.Qipi San combined with routine treatment in ICU diarrhea patients with cold-heat complex syndrome has better clinical effectiveness than routine treatment.2.Qipi San can improve the degree of diarrhea in ICU patients with cold-heat complex syndrome,worthy of broader application in clinical practice.3.Qipi San can decline the illness severity of ICU diarrhea patients with cold-heat complex syndrome.
