Functional dyspepsia (FD) is a common digestive disease, which could occur at any age group, and female more than male. It gets more and more attention because of its symptoms, poor specificity, longer duration, high recurrence rate, and seriously affecting the quality of life of patients, significantly increasing financial and psychological burden of patients. Oral drugs consist of gastrointestinal dynamic drugs, antacids, digestive enzymes and anti-anxiety drugs are main means of FD treatment, but there are still many problems exists, like high recurrence rate during discontinuation, poor long-term drug efficacy and more side effects. However, traditional Chinese medicine (TCM) bases on syndrome differentiation with individualized, holistic treatment ideas, for both specimens, chich is worth further exploration and promotion by its stable efficacy and fewer side effects.The thesis is divided into two parts, literature review and clinical research.Part1:Literature review1. Introduction of western medical progress about functional dyspepsia from the following parts:risk factors, pathogenesis, diagnosis and classification criteria, examination methods and treatments.2. TCM overview of the name, etiology and pathogenesis, syndrome differentiation and treatments of functional dyspepsia.Part2:Clinical researchObjective:Compare clinical effect of Weikangning decoction and trimebutine maleate capsules on cold and heat mixed type of functional dyspepsia.Methods:The randomized, controlled study randomly divided60cold and heat mixed type of functional dyspepsia patients into treatment group and control group.30patients in Chinese medicine group took Weikangning decoction, while the other30patients in western medicine group took maleate capsules. Treatment course of both group were4weeks. Clinical symptom scores were recorded and analyzed.Results:A total of59patients completed the clinical observation,30patients in Chinese medicine group and29patients in western medicine group.1patient lost to follow-up in western medicine group was not included in statistical analysis. Clinical efficacy form shows: Total efficacy of Chinese medicine group and western medicine group at two weeks were76.67%and58.62%respectively, and the remarkable efficacies were16.67%and3.45%respectively. The both difference were not statistically significant (P>0.05); Total efficacy of Chinese medicine group and western medicine group at four weeks were96.67%and82,76%respectively, and the remarkable efficacies were16.67%and10.34%respectively. The both difference were not statistically significant (P>0.05).Cold and heat mixed syndrome symptom score grading quantization table shows:Total efficacy of Chinese medicine group and western medicine group at two weeks were70.00%and51.72%respectively, and the remarkable efficacies were0%and3.45%respectively. The both difference were not statistically significant (P>0.05); Total efficacy of Chinese medicine group and western medicine group at four weeks were93.33%and68.97%respectively, and the remarkable efficacies were0%and3.45%respectively. The both difference were statistically significant (P<0.05). Clinical efficacy form shows:For two groups, total symptom scores and each single symptom score at all observation time points had improved compared with the scores before treatment (P<0.05).Scores of Chinese medicine of clinical symptoms at two weeks better than western medicine group, improvement was statistically significant difference between the two groups at four weeks (P<0.05)。Scores of epigastric burning sensation for Western medicine group were better than Chinese medicine group, and the symptom scores had significant improvement (P<0.05); but scores of early satiety and postprandial discomfort for Chinese medicine group were better than Western medicine group, and the symptom scores had significant improvement (P<0.05). For Chinese medicine group, score of early satiety and postprandial discomfort had significant improvement at two weeks and four weeks (P<0.05), While for Western medicine group, score of upper abdominal pain had significant improvement at four weeks (P<0.05), score of epigastric burning sensation had significant improvement at both two weeks and four weeks (P<0.05). FDDQL table rating analysised:total scores at all observation time points had improved compared with the scores before treatment (P<0.01); total scores of two groups were statistically significant (P<0.01) also. For Chinese medicine group scores of anxiety, diet, discomfort, healthy feeling, stress and disease control at two weeks are better than western medicine group, the difference was statistically significant (P<0.05), and in anxiety, diet, healthy feeling, stress and disease control at four weeks are better than western medicine group, the difference was statistically significant (P<0.05).Cold and heat mixed syndrome symptom score grading quantization table shows:total scores at all observation time points had improved compared with the scores before treatment (P<0.05); scores of two groups at2weeks had no significant improvement (P>0.05); scores of two groups had statistically significant (P<0.05) at4weeks.Conclusion:Weikangning decoction has good therapeutic effect on cold and heat mixed type of functional dyspepsia. The study shows that it is rapid onset, lasting effect, significantly relieving symptoms of functional dyspepsia and effectively relief overall discomfort of patient, improving their quality of life. Also, its none significant side effects and safety are worthy to be further research and extension in the father.
