Background:Patients with Alzheimer's disease(AD)often combined with the pathological changes such as thickness of retinal nerve fiber layer(RNFL),decreased retinal vessel density,decreased retinal vessel curvature,and increased arteriovenous curvature.However,the potential value of eyes collateral channels characteristics(similar to bulbar conjunctiva microcirculation)in the diagnosis and treatment of AD hasn't been explored.Objective:Modern technology was used to explore the shape,color and location characteristics of eyes collateral channels in patients with AD,and analyzed their correlation with TCM syndromes.This paper explored the potential value of ocular features in the early diagnosis and intervention of AD.Method:According to the diagnosis criteria,subjects from the memory clinic were divided into AD group and normal cognitive(NC)group.The subjects were assessed by neuropsychology test and scored by TCM syndrome elements,and their eyes images were collected by the ophthalmic instrument to extract the digital results of eyes shape,color and location characteristics of eyes collateral channels.The important features of eyes collateral channels and the correlation with TCM syndromes in AD group were analyzed.Result:A total of 220 subjects were enrolled,including 128 in AD group and 92 in NC group.The average age of AD group was 72.34±9.11 years old,and NC group was 67.63± 10.82 years old.The results showed that TCM syndromes in AD group were mainly distributed in marrow depletion(23.44%),phlegm turbidity(21.09%),and kidney deficiency(14.06%).The syndrome elements score of kidney deficiency in AD group(3.29±3.25)was higher than that in NC group(2.08±2.51)(P=0.002),and the score of marrow depletion in AD group(3.35±3.64)was higher than that in NC group(2.27±2.75)(P=0.013).The important shape characteristics of eyes collateral channels in AD group was“spot”,and the feature score(21.13±11.86)was higher than that in NC group(17.93± 10.13)(P=0.038).The important location characteristics of eyes collateral channels in AD group was area“E”(above the iris,corresponded the kidney and bladder),and the feature score(7.18±5.68)was higher than that in NC group(5.72±4.59)(P=0.036).The important color characteristics of eyes collateral channels in AD group was“dark brown”,and the feature score(8.91 ±7.99)was higher than that in NC group(5.72±6.15)(P=0.001).In AD group,the syndrome elements score of kidney deficiency was negatively correlated with the feature score of "spot"(r=-0.213,P=0.016)and the feature score of "dark brown"(r=-0.184,P=0.038)Conclusion:The main syndrome of patients with AD were kidney deficiency,marrow depletion and phlegm turbidity.The important shape characteristics of eyes collateral channels was " spot",indicating phlegm turbidity and dampness stasis syndrome,the important color characteristics of eyes collateral channels was "dark brown",indicating phlegm turbidity and blood stasis syndrome.Both of them reflect "solid superficial" in the pathogenesis of AD.The important location characteristics of eyes collateral channels was area "E",which corresponded the "kidney and bladder",indicating the core pathogenesis of AD was"deficiency essential" in the kidney.These initial findings had implications for the diagnosis and TCM syndrome differentiation of AD.
