With the popularization and innovation of thoracic CT technology,the detection rate of pulmonary nodules is increasing day by day.As a result,more and more patients are clinically treated for pulmonary nodules.At the same time,the 5-year survival rate of lung cancer patients is only 17%.It is an important reason that the diagnosis of lung cancer is in the middle and late stages.Therefore,more and more attention has been paid to the study of early lung cancer,and the focus of the study of lung cancer has gradually moved forward to the stage of pulmonary nodules.In recent years,many diagnostic and therapeutic techniques for early lung cancer have emerged in the field of Western medicine and modern science.Traditional Chinese medicine has a relatively mature and perfect theoretical system for lung cancer,and has a lot of clinical treatment experience,but the understanding of pulmonary nodules is still relatively small.Considering that the patients with pulmonary nodules have atypical clinical symptoms,and the theory of constitution in TCM theory can be used to describe the state of human body under the conditions of disease and health respectively,this study is conducted from the perspective of TCM constitution.In recent years,it has been found that the distribution of TCM constitution in lung cancer patients has certain characteristics,and TCM constitution is also related to TCM syndromes and clinical stages of cancer.The purpose of this study is to investigate the constitution of patients with pulmonary nodules with complete clinical imaging data,and to explore the constitutional characteristics of patients with pulmonary nodules by observing the risk factors and constitutional distribution of traditional Chinese medicine.ObjectiveTo explore the constitutional characteristics of Chinese medicine in patients with pulmonary nodules.MethodThis study investigates the constitution of patients with pulmonary nodules in the outpatient department of respiratory department of Dongzhimen Hospital,Beijing University of Traditional Chinese Medicine.From January 2018 to January 2019,a total of 86 patients with solitary pulmonary nodules were enrolled and completed the Chinese medicine constitution questionnaire.SPSS20.0 statistical software was used to analyze the data.Qualitative data were expressed by percentage.Chi-square or Fisher's exact probability method was used for comparison between groups.Quantitative data were measured by mean(+standard deviation),independent t-test was used for comparison between groups,and multivariate logistic regression was used for analysis.P<0.05 showed significant difference.FindingsAccording to the criteria of the Society of Geriatrics of the Chinese Medical Association,age was divided into two groups:the young and middle-aged group(30-59)and the old group(60-89).The proportion of males in the 30-59 age group is 56.4%,43.6%in the 60-89 age group,and 51.1%in the 30-59 age group and 48.9%in the 60-89 age group.There is no difference between male and female age groups.Among all pulmonary nodules,29(33.7%)occurred in the upper lobe of the right lung,19(22.1%)in the middle lobe of the right lung,9(10.5%)in the lower lobe of the right lung,13(15.1%)in the upper lobe of the left lung,and 16(18.6%)in the lower lobe of the left lung.Of all the patients with pulmonary nodules,2(2.3%)had thoracic CT findings with burr sign.Among all 86 cases of pulmonary nodules,50 were solid nodules,29 were partial solid nodules and 7 were pure ground glass nodules.According to the diameter of the nodules,there were 17 small nodules(diameter less than 5 mm),47 small nodules(diameter 5-10 mm)and 22 pulmonary nodules(diameter 10-15 mm)with diameter of 10-30 mm.ConclusionAmong all the solitary pulmonary nodules included in this study,29 cases(33.7%)were Yang deficiency,17 cases(19.8%)were placid,12 cases(14.0%)were phlegm-dampness,7 cases(8.1%)were Yin deficiency,5 cases(5.8%)were damp-heat,5 cases(5.8%)were Qi depression,4 cases(4.7%)were Qi deficiency and 3 cases(3.5%)were blood stasis.Yang deficiency,phlegm-dampness and yin deficiency are the susceptible constitutions of pulmonary nodules.
