Based On Data Mining, Xu Runsan And His Disciple Wang Qing Treat Dysmenorrhea Syndromes And Medication Rule Research

Objective:Based on the assistant platform of traditional Chinese medicine inheritance(TCMISS V2.5),data mining was used to conduct a retrospective study on the clinical cases of dysmenorrhea treated by the Chinese medicine master Xu Runsam and his successor professor Wang Qing,to analyze the classification of sexual taste and menstruation,high-frequency drugs and efficacy,and to explore the dialectical characteristics and medication rules of treating dysmenorrhea by Xu and his successors.At the same time,the similarities and differences in the treatment of dysmenorrhea between Xu and his successors were compared to explore the innovation of the school of gynecology in the inheritance of Xu.Methods:The study was conducted to collect the prescriptions of patients with dysmenorrhea who visited professor Xu Runsan’sclinic from January 1,2012 to November 1,2019,and professor Wang Qing’sclinic from January 1,2015 to November 1,2019.By using the traditional Chinese medicine inheritance auxiliary platform to input data systematically,the author mined its sexual taste and meridian,high-frequency drugs,prescription rules,core prescriptions and potential new prescriptions,and analyzed the excavated resultsResearch results:(1)According to the statistical analysis of the age of patients,the patients included by the two professors were the most aged patients between 26 and 35 years old,accounting for 35%,the patients aged between 26 and 45 years old accounted for more than 65%of the whole,and the patients in professor Wang’s group were younger.(2)Professor Xu’s TCM syndromes of primary dyspenorrhea were mainly kidney deficiency and liver stagnation syndrome,qi deficiency and blood stasis syndrome,kidney Yang deficiency and decline syndrome,qi stagnation and blood stasis syndrome,followed by cold coagulation and blood stasis syndrome,dampness and heat stasis syndrome,spleen and stomach deficiency and cold syndrome,blood stasis syndrome,qi and blood deficiency syndrome,Yang deficiency and internal cold syndrome,liver stagnation and spleen and kidney deficiency syndrome,and phlegm and blood stasis syndrome.Professor Wang’s TCM syndromes of primary dyspenorrhea were cold coagulation and blood stasis syndrome,liver stagnation and kidney deficiency syndrome,qi stagnation and blood stasis syndrome,followed by cold dampness syndrome,qi deficiency and dampness syndrome,Yang deficiency and internal cold syndrome,blood deficiency and blood stasis syndrome,qi and blood deficiency syndrome,Yin deficiency and fire flourishing syndrome,spleen and kidney deficiency syndrome,liver stagnation and spleen deficiency syndrome and kidney deficiency and blood stasis syndrome.(3)In the four aspects of qi,the two professor medication are temperature,cold,flat-based.In terms of five tastes,professor Xu used medicine mainly to xin,bitter and sweet,while professor Wang used mainly to bitter,sweet and sweet.and meridian attributable mainly to the liver,spleen,kidney and heart.(4)In terms of high-frequency drugs,the high-frequency drugs used by professor xu are angelica sinensis,chaihu,chuanxiong,motherwort,radix astragali,ziheche,red peony root,salvia miltiorrhiza and cornus officinalis,respectively.The high frequency medicine that professor wang uses in turn is red peony root,licorice root,astragalus root,salvia miltiorrhiza,zedoary,fu ling,angelica root,notoginseng powder,cassia twig,chai hu.(5)In terms of drug efficacy,the proportion of tonic drugs in xu’s group was 26.84%,the proportion of blood-activating drugs was 21.24%,and the proportion of dispersive drugs was 6.01%.In professor Wang’s gtoup,blood-activating drugs accounted for 19.03%,tonic drugs accounted for 16.77%,dispersive drugs accounted for 7.86%,and heat-clearing drugs accounted for 5.07%.(6)In terms of drug dose and the number of prescriptions,the number of prescriptions in professor Xu’s group was small and the quantity of prescriptions was large,with an average of 7-11 prescriptions per TCM,while that in professor Wang’s group was 12-15 prescriptions per TCM.(7)Association rules were used to analyze the drug use patterns:professor Xu’s commonly used drug patterns:bupleur-angelica-ziheche,bupleur-angelica-leonurus,xiangfu-ziheche-leonurus,angelica-ziheche-xiangfu,bupleur-ziheche-xiangfu,etc.While professor Wang’s commonly used drug model is:red peony root-astragalus-licorice,red peony root-zedoary-licorice,cassia twig-fu ling-peony skin,red peony root-licorice-notoginseng powder.(8)Potential new recipe was found by cluster analysis:one new recipe was found by professor Xu’s group:astragalus root,radix paeoniae root,rhizoma cymbiae rhizome,rhizoma cymbiae rhizome.Professor Wang group found two newrecipes:red peony,white peony,dodder,motherwort;Cassia twig,tuckahoe,peony skin,peach kernel,bupleurum.Conclusion:(1)Professor Xu’s treatment of dysmenorrhea can be divided into three categories:false syndrome,false and solid mixed syndrome,and empirical evidence.The syndrome of kidney deficiency and liver stagnation,qi deficiency and blood stasis,kidney Yang deficiency and decline,and qi stagnation and blood stasis are commonly seen.The medicine is warm,cold and flat,and the main ingredients are xin,bitter and sweet,and the channels of attribution are liver,spleen,kidney and heart.The medication modes are tonifying kidney,soothing liver,nourishing blood and activating blood,regulating menstruation and relieving pain.The core prescription is:bupleurum chinensis,angelica sinensis,ligusticum chuanxiong,ziheche,motherwort,xiangfu.From the medication pattern and the legislation of the core prescription,professor Xu’s treatment of dysmenorrhea in the deficiency and diarrhea,more emphasis on the side of deficiencyProfessor Wang’s treatment of dysmenorrhea can be divided into three categories positive evidence,false syndrome,and false and solid mixed syndrome.The common syndromes are cold coagulation and blood stasis,liver stagnation and kidney deficiency,qi stagnation and blood stasis,and phlegm and blood stasis.The medicine is warm,cold and flat,and the main ingredients are xin,bitter and sweet,and the channels of attribution are liver,spleen,kidney and heart.The modes of drug use are:wen jing dispersing cold,tonifying qi nourishing blood,promoting blood circulation and relieving pain.The core prescription is:cassia twig,poria cocos,peony skin,red peony root,licorice root,chaihu root,astragalus root,salvia miltiorrhiza,panax notoginseng powder,zedoary,pu huang,five ling zhi.According to the medication pattern and the legislation of the core prescription,professor Wang emphasizes more on the side of diarrhea in the treatment of dysmenorrhea.(2)The non-menstrual treatment of patients with dysmenorrhea in professor Xu’s group was often treated from the perspective of liver stagnation and kidney deficiency or kidney Yang deficiency and failure.The medication mode is kidney tonifying,liver soothing,air regulating,and the core prescription is tiaochongfang.Menstrual period is often treated from the perspectives of qi deficiency and blood stasis,qi stagnation and blood stasis,among which qi deficiency and blood stasis are more common.The mode of medication is to invigorate qi and promote blood circulation,regulate menstruation and relieve pain.The core prescriptions were huangqi,chishao,chuanxiong,ezhu and sanqifen.For acquired dysmenorrhea caused by adenomyosis and acute or chronic pelvic inflammatory disease,were treated from the perspective of qi stagnation blood stasis.The mode of medication is to promote qi and blood circulation,remove blood stasis and relieve pain,and the core medication is tongluo decoction plus or minus.The treatment of non-menstrual dysmenorrhea in professor Wang’s group was consistent with that of professor Xu,from the perspective of liver stagnation and kidney deficiency for treatment,and the core prescription is tiaochongfang.Menstrual period is mostly empirical,often from the perspective of cold coagulation blood stasis,qi stagnation blood stasis,phlegm stasis and other aspects of treatment.Among them,the medication mode of cold coagulation and blood stasis syndrome is wen jing dispersing cold,removing blood stasis and relieving pain.The core prescription is wen jing decoction with shixiaosan or guizhifulingwan with shixiaosan.The medication mode of qi stagnation and blood stasis syndrome is activating qi and removing blood stasis and relieving pain.The core prescription is sinisan,For acquired dysmenorrhea caused by adenomyosis and endometriosis,professor Wang often discusses the treatment from the perspective of mutual phlegm and blood stasis.The core prescription is guizhifulingwan with xiaoluowan.
