The Clinical Study Of Simiao Pills In The Treatment Of Scalp Seborrheic Dermatitis (damp-heat Syndrome)

Seborrheic Dermatisis(SD),also known as seborrheic eczema,has clinical manifestations of erythema,pimples,and scaly.It occurs in areas rich in sebaceous glands such as the head,face,and trunk,and can be accompanied by varying degrees of itching.It is a common Skin disease.The incidence of adults is 2%-5%.The etiology and pathogenesis of SD are not yet clear.Modern research believes that it is related to Malassezia furfur infection,hormone levels,epidermal barrier function,immune regulation and other factors.The esterase secreted by Malassezia can break down sebum,and the inflammatory factors produced can cause abnormal skin keratosis and inflammatory response,thereby destroying the skin barrier function.The disease is easy to diagnose and has good short-term treatment effects,but it is easy to relapse,which seriously affects patients’ quality of life and mental health.It is a refractory recurrent disease.At present,the commonly used treatment methods in modern medicine include drug therapy,which mainly includes antifungal drugs,calcineurin inhibitors,and corticosteroids;physical therapy includes narrow-spectrum medium-wave ultraviolet(NB-UVB),intense pulsed light(IPL),and radio frequency(RF),photodynamic therapy(PDT),etc.The drug treatment method is single and has large side effects,which is not suitable for long-term use.The scope of application of physical therapy is limited,and the therapeutic effect has not been clarified.Therefore,research on the treatment of SD has important scientific and clinical value.Traditional Chinese medicine has a long history of treating seborrheic dermatitis.There are a variety of treatment methods such as oral and external Chinese medicine,acupuncture,and thread embedding.The efficacy is safe and reliable,and the side effects are small.It is easy to be accepted by patients.Seborrheic dermatitis belongs to the range of "face oil wind"and "white dandruff wind" in traditional Chinese medicine.Damp heat is the main pathogenesis of the disease.Erythema papular scales are the manifestation of hot and humid flow on the skin.In addition,patients with seborrheic dermatitis often have a series of moist heat symptoms such as sticky stools or constipation,yellow urine,bad breath,yellow tongue and greasy fur,and slippery pulses.Through continuous clinical exploration,we have found that the Chinese medicine Simiao Wan is effective in treating scalp seborrheic dermatitis(damp-heat syndrome).Simiao Wan is derived from "Easy to Read" and is flavored by Ermiao San.The prescriptions include Cangzhu,Cork,Achyranthes bidentata,Coix seed,which has the effect of clearing heat and dampness.The application of Simiao Wans to the treatment of damp-heat type scalp seborrheic dermatitis can not only improve the local skin symptoms,but also improve the patients’ damp-heat symptoms,improve the quality of life,reduce the recurrence rate,alleviate the pain of patients,achieve the purpose of "upper wetness and lower treatment".This research will provide new ideas for the treatment of seborrheic dermatitis,and will have certain significance for the development of modern Chinese medicine.Objective:To explore the safety and effectiveness of Simiao Wans in treating scalp seborrheic dermatitis(damp-heat syndrome),and to make a tentative analysis of its mechanism of action,in order to provide the theoretical basis for Simiao Wan to treat scalp seborrheic dermatitis.Methods:A self-contrast controlled study was used to treat 35 patients with seborrheic dermatitis of the head who were eligible for the study by oral administration of Simiao Wan for eight weeks.By recording the general information of the patients before and after treatment,the clinical lesion score,and the TCM damp-heat syndrome score,Fixed scalp sebum rate,peripheral blood cell inflammatory factors and imaging data were compared before and after to evaluate the efficacy.Results:After 8 weeks of oral Simiao Wan treatment,the scores of clinical skin lesions and TCM dampness-heat syndrome scores of the subjects were significantly lower than before treatment,and the difference was statistically significant(P<0.01).After treatment,the sebum rate in the forehead hairline and Baihui acupoints of the patients was significantly lower than before treatment,and the difference was statistically significant(P<0.01).After treatment,the peripheral blood interleukin 8(IL-8)concentration was significantly lower than that before treatment,and the difference was statistically significant(P<0.01).After treatment,the levels of uric acid and creatinine in the peripheral blood of the subjects were lower than those before treatment,and the difference was statistically significant(P<0.05).The total effective rate reached 70.97%.Conclusion:Simiao Wan can significantly improve the symptoms of skin lesions and symptoms related to damp-heat in scalp seborrheic dermatitis in patients with damp-heat syndrome;it can reduce the rate of scalp sebum in patients,reduce oil leakage;The levels of uric acid and creatinine suggest that the IL-8 factor,uric acid and creatinine may be objective indicators related to "damp heat".And the drug has high safety during the treatment cycle,and there is no obvious abnormality in blood routine and liver and kidney function.Simiao Wan is a safe and effective method for the treatment of scalp seborrheic dermatitis.The treatment mechanism may be to reduce the secretion of oil,inhibit the secretion of inflammatory factors,and can significantly improve the symptoms of damp heat in patients.
