The Relationship Between Polysomnography And TCM Syndromes In Patients With Chronic Insomnia, And The Evaluation Of The Curative Effect Of Acupuncture At Sishen Group Points

Objective:To study the characteristics of polysomnogram of chronic insomnia patients by comparing the parameters of polysomnogram before and after acupuncture between chronic insomnia group and healthy control group,chronic insomnia group with deficiency and demonstration of traditional Chinese medicine,chronic insomnia group,and chronic insomnia group,and to compare whether there are differences in sleep process,sleep structure and special brain waves between the deficiency and demonstration of insomnia and before and after acupuncture of "Sishen point".Methods:According to the classification standard of TCM syndrome types,the insomnia patients were divided into two groups:deficiency syndrome group and positive syndrome group.Then all the subjects were monitored with polysomnography for two consecutive nights,and the polysomnography parameters of the next night were used.After sleep monitoring,all patients with insomnia were treated with acupuncture 3 times a week and 18 times a week.After all the acupuncture treatment,the patients with insomnia were monitored with polysomnography.At last,the differences of polysomnogram parameters between insomnia group and healthy control group,insomnia group before and after acupuncture were analyzed.Results:(1)According to the diagnosis and inclusion criteria,26 patients with chronic insomnia and 25 healthy subjects were included.There were 11 insomnia patients with syndrome differentiation and demonstration and 15 insomnia patients with deficiency syndrome.There was no significant difference in age and gender between groups(P>0.05).(2)Compared with the normal range,the total sleep time and sleep efficiency of polysomnography in insomnia group and control group were lower than the normal range,sleep latency,awakening times,N1%were higher than the normal range,and N2%and REM%in control group were higher than the normal range.(3)There was no significant difference in bed time,total sleep time,sleep efficiency,sleep latency,N1 duration,N3 duration,REM duration,total wake-up time,N1%and N3%between the insomnia group and the control group(P>0.05);the duration of N2,N2%,number and density of spindles in the insomnia group were lower than those in the control group(P<0.05):the difference was statistically significant(P<0.05).(4)The number and density of wakefulness,REM and ?wave in sleeping group were significantly higher than those in control group(P<0.05).The sleep latency of insomnia patients in the empirical group was significantly higher than that in the deficiency syndrome group,and the number and total time of awakening were significantly lower than that in the deficiency syndrome group(P<0.05);there was no significant difference in other PSG parameters between the two groups(P>0.05).(5)The sleep efficiency,the duration of N2 phase,the number and density of spindles increased significantly(P<0.05),while the other PSG parameters did not change significantly(P>0.05).Conclusion:The patients with chronic insomnia had a decrease in the duration of N2 phase,an increase in the percentage of REM,an increase in the number of wakes at night,a decrease in the number and density of spindles,and an increase in the number and density of delta waves.The sleep latency of patients with chronic insomnia is longer than that of patients with deficiency syndrome,that is,it is more difficult to fall asleep;the number and total time of awakening at night of patients with deficiency syndrome is significantly more than that of patients with demonstration syndrome,that is,they are more likely to wake at night;acupuncture at "Sishen point" can effectively improve the sleep efficiency of patients with chronic insomnia,which may be caused by regulating the expression of various neurotransmitters in the brain,affecting the sleep of thalamic reticular nucleus-The regulation of awakening cycle.
