ObjectiveGastroesophageal reflux asthma,as one of the extra-esophageal symptoms of gastroesophageal reflux disease,involves multiple systems and has complicated clinical manifestations.Western medicines currently in clinical use have limitations for the treatment of this disease.Therefore,on the basis of the previous research on acupuncture at the dorsal Du meridian for gastroesophageal reflux disease,and based on the theory of "different diseases and the same treatment" of traditional Chinese medicine,we still acupuncture below the Spinous process from T3 to T12 on the dorsal segment of the Du meridian to treat gastroesophageal reflux asthma,and compared with the western medicine group taking omeprazole enteric capsules.The aim was to evaluate the clinical effect and the quality of life of patients with gastroesophageal reflux asthma treated with acupuncture below the Spinous process from T3 to T12 on the dorsal segment of the Du meridian,and to observe the changes in acupoint pain threshold before and after acupuncture intervention to enrich the theory of acupuncture "taking tenderness as acupoints".MethodsCollect 60 patients who met the inclusion criteria in the acupuncture department of Beijing Guangji Chinese Medicine Hospital from January 2018 to August 2019,and divide them into acupuncture group and western medicine group according to the order of treatment by using random number table method,30 cases per group.In the acupuncture group,the course of disease was 1 year to 38 years,with an average of(8.12±9.14)years;aged 25-70 years,with an average of(54.53±13.18)years;there were 11 males and 19 females.In the Western medicine group,the course of disease was from 1.5 years to 50 years,with an average of(8.98±9.97)years;aged 33-73 years,with an average of(53.27 ± 11.42)years;there were 13 males and 17 females.There was no statistical difference in gender,age,course of disease and other baseline data,between the two groups(P>0.05),which was comparable.Both groups were given standard anti-bronchial asthma treatment,using budesonide formoterol powder inhalation(Shanghai Xinyi Bailuda Pharmaceutical Co.,Ltd.,National Pharmaceutical Standard H20080316),1 inhalation/time,2 times/d.When asthma exacerbates,inhale salbutamol(Shanghai Xinyi Pharmaceutical Co.,Ltd.,National Pharmaceutical Standard H20153141)100?200?g.The western medicine group added oral omeprazole enteric-coated capsules on the basis of standardized anti-bronchial asthma treatment,20 mg each time,twice a day for 8 weeks.In the acupuncture group,the "dredge Du meridian to lower adverse Qi" method was adopted on the basis of the standard anti-bronchial asthma treatment,that is,acupuncture below the Spinous process from T3 to T12 on the dorsal segment of the Du meridian,3 times a week,on Tuesday?Thursday and Saturday morning,8 weeks as a course of treatment.Through the use of RDQ questionnaire score,asthma control test,GERD secondary and extraesophageal symptom score,point tenderness and pain threshold detection and other methods to evaluate the efficacy of acupuncture and Western medicine.result1.By comparing the RDQ before and after treatment between the two groups of GERA patients,we found that in the acupuncture group,3 cases were cured,11 cases were markedly effective,14 cases were effective,2 cases were ineffective,and 0 cases were worse.The cure rate of the acupuncture group was 46.67%.In the western medicine group,one case was cured,no one was markedly effective,7 cases were effective,22 cases were ineffective,and no one was worsened.The cure rate of the western medicine group was 3.33%.According to statistical analysis and comparison,the effect of acupuncture group is better than the effect of western medicine group(P<0.05).The RDQ scale scores of patients in the acupuncture group and the Western medicine group both reduced after treatment(P<0.05),and the RDQ scale scores of the acupuncture group after treatment were significantly lower than those of the Western medicine group(P<0.05).2.Compare asthma control tests before and after treatment between the two groups of patients we found that there were 3 cases of complete control of asthma in the acupuncture group,16 cases of well-controlled asthma,and 10 cases of uncontrolled asthma.The rate of asthma control in the acupuncture group was 66.70%.There were 0 cases of complete control of asthma in the Western medicine group,4 cases of well-controlled asthma,and 26 cases of uncontrolled asthma.The asthma control rate in the Western medicine group was 13.33%.After statistical analysis and comparison,the asthma control rate of the acupuncture group was better than that of the western medicine group(P<0.05).Both the ac upuncture group and the western medicine group had an increased asthma control test score after treatment(P<0.05).After treatment,the acupuncture group asthma control test score was obviously higher than the western medicine group(P<0.05).Comparing the difference of ACT score between the acupuncture group and the western medicine group before and after treatment,the acupuncture group was higher than the western medicine group(P<0.05).3.Comparison of the GERD secondary and extraesophageal symptom scores of the two groups of GERA patients found that among the 30 patients in the acupuncture group and the Western medicine group,the number of cured and markedly effective was 0,the effective number was 27,the invalid number was 3,in the acupuncture group,and the number of cured and markedly effective was 0,the effective number was 3,the invalid number was 27,in the western medicine group.The total effective rate in the acupuncture group is 90%,and the total effective rate in the western medicine group is 10%.Compared with the western medicine group,the scores of digestive system symptoms,respiratory symptoms,and sleep disturbances in the acupuncture group were significantly reduced(P<0.01);the scores of facial features symptoms,and poor mood after the acupuncture group were lower than before treatment(P<0.05);only facial features symptoms scores in the Western medicine group decreased after treatment than before treatment,and the acupuncture group improved the above symptoms significantly better than the Western medicine group.There was no significant difference in the improvement of circulatory symptoms between the acupuncture group and the Western medicine group(P>0.05).In the acupuncture group,the total score of after treatment was lower than the total score before treatment(P<0.05);the difference of the total score before and after treatment in the western medicine group was not statistically significant(P>0.05).4.Analysis and comparison of T3 to T12 Spinous process tenderness in the dorsal segment of Du meridian in the two groups of GERA patients.Only T3 and T11 had less tenderness under spinous processes than before treatment(P<0.05).After treatment,the degree of tenderness improvement in the acupuncture group was obviously better than that in the Western medicine group.5.The T3 to T12 Spinous process tenderness thresholds of the dorsal segment of the Du meridian before and after treatment in two groups of GERA patients were compared within the group.Thresholds were higher than before treatment(P<0.05).Western medicine group only had T6,T7 and T9 Spinous process tenderness thresholds were higher than before treatment(P<0.05);after treatment between groups,the acupuncture group was higher than the western medicine group at T3 to T12 Spinous process tenderness thresholds(P<0.05).In the acupuncture group,the T3 to T12 Spinous process tenderness thresholds were significantly better than the western medicine groupconclusion1.The asthma control test results and RDQ scores of this study showed that the control rate of asthma and the healing rate of RDQ were obviously better than that of the Western medicine group in the treatment of GERA with acupuncture at the back of Du meridian and the Western medicine group taking omeprazole enteric capsules.The improvement of tenderness of T3 to T12 Spinous process tenderness in the dorsal segment of Du meridian is also significantly better than that of the Western medicine group.It shows that acupuncture at the dorsal segment of Du meridian can not only effectively control asthma and relieve reflux symptoms,but also alleviate the accompanying symptoms of various systems,reduce the waste of medical resources,and reduce the pain and economic burden of patients2.The comparison of T3 to T12 Spinous process tenderness thresholds of the dorsal segment of the Du meridian before and after treatment in two groups of GERA patients also showed that the acupuncture improved the T3 to T12 Spinous process of GERA patients better than Western medicine group.Exploring the tender reaction points below the spinous processes of T6 to T9 of Du meridian can assist in the diagnosis of GERA.
