Research Background:Knee osteoarthritis(KOA)is a degenerative disease of the joint and it is widespread in incidence,which seriously affects the motor function of elderly people and even causes disability.there is no cure method.Inflammatory pain in the joint or joint deformity,will result in degeneration of the muscle structure and function around the joint,and further aggravate the patient's dysfunction.Exercise intervention is an important method of adjuvant therapy,which has an important influence on improving muscle function around joint,and it can also promot the joint's stability and reduce the disability.At present,some scholars have found that the fat content of medial femoral muscle may be a new independent risk factor to predict the structure and function of KOA patients.there may be a correlation between intramuscular fat content and the strength,power and motor function of the KOA patient.Intramuscular fat content represents the severity of fatty infiltration of muscle contractile structure and it is a microstructural indicator for reducing muscle function.High speed power training has been reported that it can improve muscle function,but no studies have focused on the effect of the exercise intervention on intramuscular fat.These studies have a great significance for the development of teh effective KOA exercise prescriptions,and it can be a guide for KOA patients' s exercise rehabilitation to reduce disability and it can also have a important significance to improve the quality of life.Research purposes:To evaluate the effect of three months' rapid strength training on KOA patients' s intramuscular fat of the medial femoral muscle using the MRI technology of quantitative analysis of fat.Research an effective KOA exercise prescription for changing muscle microstructure damage to guide the KOA exercise rehabilitation,reduce disability and improve the quality of life of patients.research method:After screening,28 subjects who met the selection criteria were included in the experiment,aged 55-70 years,and were randomly divided into experimental group(n=14)and control group(n=14).6 subjects were lost throughout the experiment,12 in the final experimental group and 10 in the control group.The exercise intervention was performed three times a week for a total of 12 weeks in experimental group,and at the same time the health education guidance was performed.The control group only had the health education guidance.To observe the effect of high speed power training on quadriceps femoris morphology and motor function in patients with knee osteoarthritis.Symptom Observation Indicator: VAS Pain Score;The shape of quadriceps femoris muscle was measured by MRI,including the percentage of intramuscula fat in the medial femoral muscle and the area of quadriceps femoris muscle;Function of quadriceps femoris indicators inclued: quadriceps muscle power(Power),quadriceps maximal voluntary isometric contraction(MVIC),The exercise ability observation indicators included :walking speed,and the climb stairs power.Research result:1.Comparison of VAS score in medial femoral muscle before and after exercise intervention: Intra-group comparison,the VAS score after exercise intervention in the experimental group was significantly lower than that before intervention(P<0.01),and the VAS scorein the control group,there was a significantly lower than that before intervention(0.01).Between groups,there was no significant difference between the experimental group and the control group before and after the VAS score(P>0.05).2.Comparison of intramuscular fat content in medial femoral muscle before and after exercise intervention: Intra-group comparison,the intramuscular fat content of the medial femoral muscle after exercise intervention in the experimental group was significantly lower than that before intervention(P<0.01),and before and after the experiment in the control group,there was no significant improvement in the fat content the medial femoral muscle(P>0.05).Between groups,there was a significant difference in the change of intramuscular fat content between the experimental group and the control group before and after the experiment(P<0.05).3.Comparison of quadriceps area before and after exercise intervention: Intra-group comparison,The area of quadriceps femoris after exercise intervention in the experimental group was not significantly higher than that before,but there was an upward trend;In the control group,after the experiment,the area of quadriceps femoris did not improve significantly,and there was a downward trend.Between groups,the difference between the experimental group and the control group before and after the quadriceps area experiment was significantly increased(P<0.05).4.Comparison of the MVIC of the quadriceps muscles before and after exercise intervention: Intra-group comparison,After exercise intervention,the MVIC of the experimental group was not significantly higher than that before,but there was an upward trend;Control group MVIC did not improve significantly,but there was a downward trend.Between groups,there was no significant difference between experimental group and control group in MVIC(P>0.05).5.Comparison of muscle power of the quadriceps muscles before and after exercise intervention: Intra-group comparison,The power of the experimental group after exercise intervention was not significantly higher than that before,but there was an upward trend;In the control group,after the experiment,the power of quadriceps femoris did not improve significantly.Between groups,there was no significant difference in power before and after the experimental group and the control group(P>0.05).6.Comparison of motor function before and after exercise intervention: Comparison of motor function before and after exercise intervention: Intra-group comparison,the climbing power and rapid pace of the experimental group were significantly improved(P<0.01),and control group climbing power significantly improved(P<0.05);In the control group,There was a significant difference in step speed before and after the experiment(P<0.05),and there was no significant difference in climbing power between the experimental group and the control group before and after the experiment(P>0.05).7.Correlation analysis found: there was a negative correlation between the extensor knee power of quadriceps femoris and intramuscular fat of medial femoral muscle(P<0.05);The maximum extensor strength(MVIC)of quadriceps femoris was not correlated with intramuscular fat(P>0.05);motor function had no significant correlation with intramuscular fat of the medial femoral muscle(P>0.05).Analysis conclusion:High speed power training reduces medial femoral muscle fat infiltration in women with knee osteoarthritis,and makes the internal structure of the skeletal muscle restored,by these ways,quadriceps muscle strength hypertrophy and motor ability can be improved.
