PurposeScientific,rational and systematic methods were used to develop a nutritional risk assessment scale for cancer patients suitable for clinical application in China,and the reliability and validity were tested and the diagnostic threshold was determined.Method1.Establish a research group to review relevant literature at home and abroad,conduct semi-structured interviews with relevant experts,and initially determine the entry of the nutritional risk assessment scale for cancer patients.2.Using the Delphi method to conduct expert consultations with physicians,nurses and nutritionists in the field of tumor-related work,and after two rounds of expert consultation,form a pre-test scale of 12 items.3.Using the convenient sampling method,276 tumor inpatients from a provincial cancer hospital were selected for investigation.The results were further screened by project analysis,factor analysis,correlation coefficient method,decision value method and discrete degree analysis to determine the final scale.The reliability and validity test of the scale was selected by internal consistency and test-retest reliability,and the content validity was used.,structural validity and calibration validity test.The diagnosis of the scale of the scale is judged by ROC analysis,and the severity of the patient's subjective overall assessment scale(PG-SGA)diagnostic criteria is used as the gold standard.Result1.The recovery rates of the two rounds of expert consultation questionnaires were 93.3% and 89.3%,respectively.The authoritative coefficients of the two rounds of experts were 0.83,and the expert coordination coefficients were 0.411 and 0.435,respectively(P<0.001).2.After reasonable statistical test,the Cronbach's ? coefficient of this scale is 0.869,the Cronbach's ? coefficient of each dimension is 0.812-0.886;the scale test reliability is 0.850;the content validity is between 0.860-1.000;Exploratory factor analysis extracted 4 common factors,and the cumulative variance contribution rate was 61.347%;the correlation coefficient between the total score of the scale and the scores of each dimension was 0.769-0.894,and the correlation coefficient between the scores of each dimension of the scale was 0.424-0.568,P<0.01;in addition,the Pearson correlation coefficient of each dimension and total score of the scale and PG-SGA was 0.431-0.759,P<0.01.3.Demarcation points: The diagnostic criteria 1,3,8,and 9 of the PG-SGA scale are used as the classification criteria for the severity of the scale,and the ROC curve is drawn to find the point where the Yoden index is the largest.The severity of this scale is divided into 0 to 2.No intervention is required at this time,and routine follow-up and evaluation are maintained during the treatment.3~5 According to the patient's symptoms and laboratory test results,the patients and their families should be educated healthily and drug intervention;6~10 According to the severity of malnutrition,reasonable nutrition intervention is carried out;?11 patients need urgently need to improve the urgent need for nutrition Intervention.ConclusionThe nutritional risk assessment scale for cancer patients compiled in this study includes 12 items in 4 dimensions including patient history,metabolic needs,physical examination and laboratory examination.The reliability and validity test is good,and it can distinguish whether the tumor patients are malnourished.It can identify the severity of malnutrition in cancer patients and provide data reference for nutritional care of clinical patients.In summary,the use of cancer patients nutritional risk assessment scale for clinical evaluation,can provide a scientific assessment tool for clinical,with good clinical practical and promotional value.
