Background:The prevalence rate of heart failure of preserved ejection fraction(HFpEF)in all of the heart failure is increasing year by year.Also,HFpEF has received more and more attention in clinical.But no drugs and treatments which have obvious effects on HFpEF and can reduce the incidence of end point events have been found.Chinese medicine treatment perhaps can play a role in it.Yiqixiefei decoction is an effective prescription for treating HFrEF,which is developed after more than 30 years of clinical experience and verification by professor Guo Weiqin,a famous Chinese doctor in Beijing.The purpose of this study was to observe whether Yiqixiefei decoction could improve the cardiac function and structure of rats with HFpEF.Objective:To observe the effect of Yiqixiefei decoction on the cardiac function and structure of HFpEF rats.Methods:SD rats were randomly divided into model animals and sham operation groups.Model animals were used to establish HFpEF models by abdominal aortic constriction.Model rats were randomly divided into model group,positive drug group(enalapril),traditional Chinese medicine group(Yiqixiefei decoction).And sham operation group is set as control group.Drug lavage began on the second day after surgery.The model group and the sham operation group were given double steaming water,the positive drug group was given enalapril,and the traditional Chinese medicine group was given Yiqixiefei decoction.After 12 weeks of lavage,the samples were taken,and the indexes are below:(1)General conditions of animals:such as spirit,behavior,reaction,hair color,appetite,hydrothorax and ascites and mortality of animals.(2)Cardiac hemodynamics:left ventricular systolic pressure(LVSP),left ventricular diastolic pressure(LVDP),left ventricular average pressure(LVAP),and left ventricular end-diastolic pressure(LVEDP).The maximum rate of left ventricular pressure rise(+dp/dtmax)and the interval between the onset of left ventricular contraction and dp/dtmax(t-dp/dtmax);The maximum rate of decrease of left ventricular pressure(-dp/dtmax)and the time constant of decrease of left ventricular pressure(T);Arterial systolic blood pressure(SP),arterial diastolic blood pressure(DP),and arterial average pressure(AP).(3)Heart and lung weight index:body weight,heart weight index,and lung weight index.(4)Ventricular structure:left ventricular wall thickness and ventricular chamber size were measured by HE stammg.(5)Myocardial fibrosis:measure collagen area ratio by Masson staining,measure the area and optical density of collagen ? by immunohistochemical determination.Results:(1)General conditions of animals:compared with the sham operation group,the model group rats were more restless,and their hair was darker and rougher;The hair color of the rats m the positive drug group and the traditional Chinese medicine group was brighter than that of the model group.No obvious hydrothorax and ascites was observed n each group.The mortality was 0%m the sham group,23.1%in the model group,38.5%in the positive drug group,and 14.3%in the Yiqixiefei decoction group(2)Cardiac hemodynamics:The heart rate of the model group was significantly higher than that of the sham group.Compared with the model group,the heart rate of the Yiqixiefei decoction group was significantly reduced(p<0.01).The heart rate of Yiqixiefei decoction group was significantly lower than that of positive drug group(p<0.01).Compared with the sham group,the left ventricular systolic blood pressure was significantly increased m the model group(p<0.05).The left ventricular diastolic pressure of the model group was significantly higher than that of the sham group(p<0.05)Left ventricular average pressure m the model group was higher than that m the sham group(p<0.05).The left ventricular end-diastolic pressure m the model group was significantly higher than that in the sham group(p<0.05).There was no significant difference among the other groups.The maximum increase rate of left ventricular pressure in the model group was significantly lower than that in the sham group(p<0.05).However,there was no significant difference in the interval between the onset of left ventricle contraction and dp/dtmax in each group.Compared with the sham group,the maximum decrease rate of left ventricular pressure m the model group was significantly decreased(p<0.05).The maximum decrease rate of left ventricular pressure in the positive drug group was higher than that in the model group(p<0.05).The left ventricular pressure drop time constant m the model group was significantly higher than that in the sham group.Compared with the model group,there was no significant difference in the left ventricular pressure drop time constant between the positive drug group and Yiqixiefei decoction group(p>0.05).(3)Heart and lung weight indexes:there was no statistical difference m body weight among all the groups;compared with the sham group,the heart weight index of the model group was significantly higher(P<0.05).Lung weight index of Yiqixiefei decoction group was significantly lower than that of model group(P<0.05).(4)Ventricular structure:the morphology of myocardial cells in the sham operation group was not significantly abnormal under light microscope.Compared with the sham group,the myocardial cells in the model group increased in diameter,length,volume and the arrangement is disordered.No obvious morphological difference was found between the model group and Yiqixiefei decoction group.The left ventricular area of the model rats increased significantly compared with the sham group(P<0.05).Compared with the model group,the left ventricular area decreased significantly in the western medicine group and Yiqixiefei decoction group(P<0.05).The left ventricular wall thickness of the model group was significantly greater than that of the sham group(P<0.05),and there was no statistically significant difference between the positive drug group,Yiqixiefei decoction group and the model group(P>0.05).(5)Myocardial fibrosis:compared with the sham operation group,the average percentage of fibrosis area in the model group was significantly increased(P<0.05).Compared with the model group,the average percentage of fibrosis area of the positive drug group and Yiqixiefei decoction group was significantly reduced(P<0.05).Conclusion:1.Yiqixiefei decoction can significantly slow down heart rate and reduce lung weight index,but has no obvious effect on diastolic function.2.Yiqixiefei decoction can significantly reduce left ventricular area.To some extent,it alleviates ventricular remodeling.3.Yiqixiefei decoction can significantly reduce the content of collagen in myocardial tissue and decrease myocardial fibrosis.
