This paper,by using 2014 and 2016 Chinese families(CFPS)tracking data, introduce the concept of spillover effects to the medical insurance safeguard function, system analysis in addition to his other out-of-court community family medical insurance situation to this family, the influence of catastrophic health care costs and then explore the existence of social medical insurance for family catastrophic health care costs spillover effect. The results show that: it is true that there is a social spillover effect in the process of guaranteeing the catastrophic medical expenditure of the family by medical insurance, and this effect can release the demand of the family's medical service in the short term, leading to the increase of the family's medical expenditure; However,in the long term,as the participation rate of other families in the community increases, the coverage of other families also increases,so that they have more funds to assist families with diseases, thus reducing the probability of catastrophic medical expenditures. In order to further verify the reliability of the results, the robustness test and the heterogeneity test of urban and rural stratification, income stratification and east and west regions were carried out in this paper, and the results are still significant. On this basis, this paper selects the occurrence of catastrophic medical expenditures of other families in the community as the mediating variable to explore the mechanism of the impact of medical insurance on the social spillover effect of catastrophic medical expenditures of families. Finally, based on the conclusion of the empirical study, the paper puts forward some policy Suggestions for further improving the multi-level medical security system in China.
