Study On The Correlation Between The Side Effects Of Chemotherapy Of Lung Cancer And TCM Syndrome Types

Abstract/Summary:
Purpose:This study aims to explore the correlation between side effects of chemotherapy and syndromes of traditional Chinese medicine(TCM)in lung cancer.Material and method:In this study,a retrospective clinical research was performed,and 73 patients were selected in accordance with the inclusion and exclusion criteria for summarizing the TCM syndromes of lung cancer patients with chemotherapy.Logistic regression analysis was carried out on TCM syndromes and side effects of chemotherapy in selected patients respectively.Fisher exact test was used to determine the frequency distribution of side effects of chemotherapy in TCM syndromes.Likewise,Fisher exact test was also used to compare the frequency distribution of the severity of bone marrow depression,types of hematopenia and types of gastrointestinal reaction in TCM syndromes of lung cancer.In addition,the frequency distribution of side effects of different chemotherapy schemes in TCM syndromes was also compared by Fisher exact test,and the significance level of the test was P < 0.05.Results:1.The distribution frequencies of TCM syndromes in 73 patients with lung cancer in chemotherapy were as follows: spleen-stomach disharmony with Yin deficiency liver-kidney(34.25%),spleen-stomach disharmony with retention of phlegm-dampness(30.14%),qi-blood deficiency(27.40%)and spleen-stomach disharmony(8.22%).Spleen-stomach disharmony combined with Yin deficiency of liver-kidney,spleen-stomach disharmony combined with retention of phlegm-dampness and qi-blood deficiency are the common syndromes in patients with lung cancer in chemotherapy.2.The side effects of chemotherapy in 73 patients with lung cancer were bone marrow suppression(49.32%),digestive tract reaction(73.97%),fatigue and lassitude(72.60%),liver function damage(20.55%),and renal function damage(8.22%).3.Logistic regression analysis was used to analyze the correlation between different syndromes and side effects of chemotherapy in 73 patients with lung cancer.The resultsshowed that there was a certain correlation among bone marrow depression,spleen-stomach disharmony combined with Yin deficiency of liver-kidney,and qi-blood deficiency(P<0.05).The frequency distribution comparison of Spleen-stomach disharmony combined with Yin deficiency of liver-kidney and qi-blood deficiency showed there were 9 cases(25%)of bone marrow depression in spleen-stomach disharmony combined with liver-kidney Yin deficiency,and 19 cases(52.78%)with bone marrow depression in Qi-blood deficiency syndrome.And fatigue was correlated with spleen-stomach disharmony and internal retention of phlegm-dampness(P < 0.05).4.Fisher exact test was used to compare the frequency distribution of chemotherapy side effects among different types of lung cancer patients.The results showed that there were no significant differences in the frequency distribution of chemotherapy side effects among spleen-stomach disharmony combined with Yin deficiency of liver-kidney and qi-blood deficiency,spleen-stomach disharmony combined with internal retention of phlegm-dampness,and spleen-stomach disharmony(P > 0.05);There was no significant difference in the frequency distribution of chemotherapy side effects of lung cancer among qi-blood deficiency and spleen-stomach disharmony,spleen-stomach disharmony,and internal retention of phlegm-dampness,and spleen-stomach disharmony(P > 0.05);the frequency of side effects of lung cancer chemotherapy showed significant differences among qi-blood deficiency and spleen-stomach disharmony and internal retention of phlegm-dampness(P < 0.05).Compared with the two syndromes,20(37.74%)cases of Spleen-stomach disharmony and internal retention of phlegm-dampness had been of fatigue,18(33.33%)cases of digestive tract reaction accounting for the highest proportion,and 19cases(52.78%)of bone marrow depression accounting for the highest proportion.5.Fisher exact test was used to compare the frequency distribution of different types of hematopenia in 73 patients with lung cancer in chemotherapy.The results showed that there was no significant difference in frequency distribution among different types of syndromes and types of hematopenia(P > 0.05).The frequency distribution of severity of bone marrow depression in 73 patients with lung cancer in chemotherapy was compared by Fisher exact test.The results showed that the frequency distribution of the severity of bone marrow depression was significantly different among Qi-blood deficiency and spleen-stomach disharmonycombined with internal retention of phlegm-dampness,spleen-stomach disharmony,and spleen-stomach disharmony combined with Yin deficiency of liver-kidney(P < 0.05).In conclusion,16 cases of degree II-IV bone marrow depression occurred in Qi-blood deficiency syndrome,with the highest frequency.6.There was no significant difference in Fisher’s exact test for the frequency distribution of gastrointestinal reactions among different syndromes(P > 0.05).7.Fisher’s exact test showed that there was no significant difference in the frequency distribution of side effects among different types of platinum-containing chemotherapy programs(P > 0.05);there was no significant difference in the frequency distribution of side effects among spleen-stomach disharmony combined with Yin deficiency of liver-kidney and qi-blood deficiency,spleen-stomach disharmony combined with internal retention of phlegm-dampness,and spleen-stomach disharmony(P > 0.05);And there was no significant difference in frequency distribution between qi-blood deficiency syndrome and spleen-stomach disharmony(P > 0.05);there was a significant difference in frequency distribution of side effects between Qi-blood deficiency syndrome and spleen-stomach disharmony combined with internal retention of phlegm-dampness(P = 0.047(P < 0.05).Comparing the two syndromes,19 cases(38%)of Spleen-stomach disharmony combined with internal retention of phlegm-dampness had been of fatigue,17 cases(34%)of digestive tract reaction accounting for the highest proportion,and 16 cases(51.61%)of bone marrow depression accounting for the highest proportion in Qi-blood deficiency syndrome.There was no significant difference in the frequency distribution of side effects among different syndromes,including etoposide combined with platinum,gemcitabine combined with platinum,and Taxus combined with platinum(P > 0.05).Conclusion:1.TCM syndromes of lung cancer patients with chemotherapy include spleen-stomach disharmony and Yin deficiency of liver-kidney,spleen-stomach disharmony and internal retention of phlegm-dampness,qi-blood deficiency and spleen-stomach disharmony;Among which spleen-stomach disharmony is the most common syndrome.2.Bone marrow depression in lung cancer patients with chemotherapy can be seen inspleen-stomach disharmony,Yin deficiency of liver-kidney and qi-blood deficiency syndrome,especially in qi-blood deficiency syndrome.As for the frequency distribution of severity of bone marrow depression in lung cancer patients with chemotherapy,degree II-IV of which are commonly seen in qi-blood deficiency syndrome.Fatigue and digestive tract reactions in patients with lung cancer in chemotherapy are commonly seen in spleen-stomach disharmony and internal retention phlegm-dampness.3.The frequency distribution of side effects in different types of platinum-containing chemotherapy programs was significantly different between Qi-blood deficiency syndrome and spleen-stomach disharmony combined with internal retention phlegm-dampness.In comparison,fatigue and digestive tract reaction were more commonly seen in spleen-stomach disharmony and internal retention of phlegm-dampness,and bone marrow depression was more commonly seen in Qi-blood deficiency syndrome.
