Purpose:To study the mechanism of acupuncturing sciatica by acupuncture Huántiào(GB30)and Wěizhōng(BL40)on rats model of sciatic nerve injury and observe the differences in the efficacy of acupuncture in term of differences of behavioral,morphological,molecular biology and expression of IL-1β,TNF-α protein.In order to provide experimental basis in term of curing sciatica in the future.Material and method: 48 SPF SD rats,6 month old,male and female,weighing 250 ± 20 g,were randomly divided into sham operation group,model group,GB30 point group and BL40 point group,and 12 rats in each group.The rat model of sciatic nerve injury was established by clamp method.After the success of the model,sham operation group and model group rats without any treatment;Rats in the GB30 group began the 2nd day after the successful modeling,and acupuncture was performed on the ipsilateral side of the rat,namely,the posterior superior edge of the hindlimb hip joint and the depression in front of the greater trochanter of the femur.After the success of the model,in second days,GB30 group and BL40 group rats was selected with muscle twitching or toe fibrillation by connection of electro acupuncture instrument with 1mA,2Hz and dilatational wave.Each treatment lasts 15 min once a day for 2w consecutively.In the BL40 group,the rats were acupunctured on the affected side of the BL40 at the 2nd day after the successful modeling,namely,the central part of the posterior limbs and the upper edge of the hip joint.SFI and MNCV were measured at first and fourteenth days after the model to evaluate the neuromuscular and motor recovery of the rats before and after treatment.The injury of sciatic nerve segments were detected after fourteenth days,besides,insciatic nerves of HE rats were detected by transmission electron microscopy staining;nerve myelin thickness;Immunohistochemistry and immunofluorescence were used to detect the expression of IL-1β and TNF-α protein in rat sciatic nerve injury,and statistical analysis was performed immunohistochemical detection of rat sciatic nerve macrophage number.Results: 1.The results of behavioral test SFI test results in each group: after first days,compared with sham operation group,SFI values in model group,GB30 point group and BL40 point group were significantly decreased(p<0.01).For 14 days,compared with the sham operation group,GB30 group and the BL40 group were significantly lower(p<0.01);The SFI evaluation of the sciatic nerve in the GB30 acupoint group was better than that of the BL40 group.MNCV test results in each group: At the first day of model establishment,compared with the sham group,the MNCV value of the model group decreased significantly(p<0.01).On the 14 th day after model establishment,compared with the model group,the MNCV values in the GB30 group and the BL40 point group increased significantly(p<0.01),and the sciatic nerve motor conduction velocity in the GB30 group was better than the BL40 group.2.The results of morphological detection The sciatic nerve HE staining results: sham operation group of HE staining showed that dense structure and sciatic nerve fibers were orderly arranged between the endoneurium of axons,myelin sheath was clear and continuous.Compared with the sham operation group,model group of rat sciatic nerve fibers arranged irregularly,axonal and myelin fracture,Schwann cell differentiation hyperplasia;arrangement of rules GB30 group and BL40 group rat sciatic nerve fibers,visible part of the nerve axons,newborn,myelin sheath shedding,including morphology of GB30 group of nerve fibers was better than the BL40.Results the thickness of myelin sheath of sciatic nerve: compared with sham operation group,model group,GB30 group and BL40 group rat sciatic nerve myelin thickness were significantly thinner(p<0.01).Compared with the model group,GB30 group and BL40 group rat sciatic nerve myelin thickness increased significantly(p<0.01),and the structure was clear and complete.So GB30 group is better than the BL40.3.Molecular biology test resultsAccording to the results of immunohistochemical detection of IL-1β,the expression of IL-1β in the model group was significantly higher than that in the sham group(p<0.05).Compared with the model group,the expression of IL-1β in the GB30 group and the BL40 group were significantly lower(p<0.05),and the expression in the GB30 group was lower than the BL40 group.According to the results of immunohistochemical detection of TNF-α,the expression of TNF-α in the model group was significantly higher than that in the sham group(p<0.01).Compared with the model group,the expression of TNF-α was significantly decreased(p<0.05)in the GB30 group and the BL40 group,and the expression level in the GB30 group was lower than that in the BL40 group.Results of immunofluorescence detection of IL-1β and TNF-α: the sham operation group showed that the sciatic nerve fibers were cord-like,regular,and only a small amount of positive expression;compared with the sham operation group,the nerve fibers of the model group were broken and disorder of running and the expression of positive substances increased significantly.The nerve fibers in GB30 group and BL40 group were more regular and the expression of positive matter was significantly decreased.The expression level of GB30 group was lower than that of BL40 group.Conclusion: 1.Acupuncture treatment can improve the motor function of the sciatic nerve and accelerate the speed of the injured sciatic nerve,and GB30 group is better than BL40 group.2.Acupuncture therapy can effectively inhibit the inflammatory reaction,promote the repair of damaged sciatic nerves,and improve its pathomorphological changes,and Acupuncture at GB30 point is better than BL40 point.3.Acupuncture treatment can obviously inhibit the protein expression of IL-1β and TNF-α inflammatory factors in the injury of sciatic nerve in rats,and GB30 group is better than BL40 group.4.The improvement of SFI and MNCV,sciatic nerve morphology,number of phagocytes and inhibition of the expression of IL-1β and TNF-α inflammatory factors in sciatic nerve injury rats may be one of the mechanisms of acupuncture promoting functional recovery of sciatic nerve injury.
