Changes In The Distribution Of Main Bacteria And The Trend Of Drug Resistance In Hospitalized Patients With Acute Exacerbations Of Chronic Obstructive Pulmonary Disease

Objective:to investigate the changes of bacterial distribution and antibiotic resistance in AECOPD patients in Beijing hospital.Methods:the sputum bacterial culture and drug sensitivity results of AECOPD patients hospitalized in Beijing hospital from January 1,2009 to December 31,2018 were retrospectively studied,and the bacterial distribution characteristics and antibiotic resistance trend of hospitalized AECOPD patients were analyzed.Results:1)a total of 3817 patients with AECOPD in 10 years,including 2861 patients who completed sputum culture for a total of 1,104 strains of various bacteria,the total positive rate of sputum culture was 38.6%(1104/2861).2)among the patients with moderate and severe COPD,the detection rate of gram-negative bacteria is relatively high,including acinetobacter baumannii,pseudomonas aeaiginosa,klebsiella pneumoniae and escherichia coli.3)in hospitalized AECOPD patients,the detection rate of pseudomonas aeruginosa using mechanical ventilation was the highest,accounting for 11.3%,followed by acinetobacter baumannii(11%)and klebsiella pneumoniae(8.8%).4)the detection rates of acinetobacter baumannii(23.1%)and staphylococcus aureus(5%)and pseudomonas aeruginosa(4.7%)were higher in patients with AECOPD hospitalized in the intensive care unit.Pseudomonas aeruginosa(21.5%),klebsiella pneumoniae(20.9%),acinetobacter baumannii(19.7%)and haemophilus parainfluenzae(11.1%)were admitted to the general respiratory medicine ward.5)with a common disease in hospitalized patients with AECOPD heavy deputy flu bloodthirsty bacillus detection significantly increased 7.2%,to merge two or more than 2 AECOPD patients in hospital is given priority to with gram-negative bacteria,including acinetobacter baumannii,pseudomonas aeruginosa,klebsiella pneumonia bacillus increased significantly,and is on the rise,gold aureus detection of glucose is on the rise,deputy h.influenzae is on the decline;6)among the hospitalized AECOPD patients with type ? respiratory failure,the major bacteria detected were acinetobacter baumannii(3.3%),pseudomonas aeruginosa(2.5%),klebsiella pneumoniae(2.1%)and haemophilus parainfluenzae(1.8%).7)pseudomonas aeruginosa(11.9%),acinetobacter baumannii(11.4%),klebsiella pneumoniae(9.1%)and haemophilus parainfluenzae(6.5%)were the most frequently detected in hospitalized patients with frequent AECOPD.8)acinetobacter baumannii,pseudomonas aeruginosa,escherichia coli,klebsiella pneumoniae,staphylococcus aureus,and streptococcus pneumoniae showed varying degrees of increasing resistance to various antibiotics.Conclusion:1)the detection rate of gram-negative bacilli in poor pulmonary function,intensive care unit,frequent attack group and number of diseases was higher.2)the antibiotic resistance rate of hospitalized AECOPD patients showed an increasing trend during 10 years.3)gram-negative bacilli are common pathogenic bacteria in patients with AECOPD,and their drug resistance is on the rise.Therefore,rational use of antibiotics is of great significance.
