Comprehensive Evaluation Of The Implementation Of Smoke-free Laws And Regulations In Cities In My Country

Background:Protecting people from tobacco smoke through smoke-free legislation is an important measure to promote the construction of smoke-free China.At present,more than 20 cities in China have implemented the smoke-free regulations,but only some cities have carried out evaluations of the implementation.The establishment of a scientific,comprehensive and feasible comprehensive evaluation index system for the implementation of smoke-free regulations is of great significance to the legislation and law enforcement of cities.Objective:The purpose of this study was to explore and initially establish a comprehensive and scientific comprehensive evaluation index system for the implementation of smoke-free laws in China,to determine the weight of indicators and to establish a comprehensive evaluation model.Through comprehensive evaluation of some legislative cities,the feasibility study and practicality of the evaluation indicators would be verified,so as to provide a reference for the formation of the final scientific and practical evaluation index system.Methods:Based on the'"six elements of successful implementation of smoke-free regulations" proposed by the World Health Organization,the basic framework for comprehensive evaluation indicators for the implementation of smoke-free regulations was constructed through literature review,personal in-depth interviews,and expert seminars.Through expert scoring method,the indicators were screened,the weights of indicators were determined.After expert discussion and research group discussion,the scoring criteria of each index were determined,and then the comprehensive evaluation model was established by comprehensive scoring method.Based on the established comprehensive evaluation index system of urban smoke-free regulations,data collection,collation and analysis were carried out for some legislative cities in China,and then the cities were scored according to the evaluation criteria of each index.Finally,the established comprehensive evaluation model was used to comprehensively score the implementation of smoke-free regulations in these cities,verified the feasibility and practicability of the indicators,and further adjusted the indicator system.Results:Through literature review,in-depth interviews,expert discussions,and two rounds of expert scoring,a comprehensive evaluation index system including 5 first-level indicators,29 second-level indicators,and 33 third-level indicators was constructed,as well as the index weights and the scoring standard were determined.The comprehensive scoring model was established by weighted accumulation.The first-level indicators included the content of regulations,law enforcement,social participation,publicity and education,and monitoring and evaluation.The weight distribution of these five parts were 0.2503,0.2400,0.1856,0.1993,and 0.1248,respectively.The positive coefficients of two rounds of expert scoring were 90%and 100%respectively;the expert's familiarity coefficients with the 5 first-level indicators were between 0.89 and 0.94,and the judgment basis coefficients were between 0.66 and 0.72.The calculated degrees of authority were between 0.78 and 0.88;the coefficients of variation of the first round expert scores of the first-level indicators were between 0.21 and 0.42,and the second round were between 0.25 and 0.32;the coordination degree of the expert opinion of the first-level indicators was 0.387{?2=27.84,P<0.05).Through the data collation and analysis of the eight legislative cities,the feasibility and practicability of the indicators were verified.Some indicators that were not practical,such as law enforcement funding and smoking ban mark qualification rate,were deleted.The evaluation criteria for some indicators were adjusted,such as the requirements for non-smoking places and staffing,making the indicator system more practical and scientific.Conclusion:After the establishment and verification of the comprehensive evaluation index system for the implementation of smoke-free regulations,an index system including 5 first-level indicators was initially established.The research results showed that the comprehensive evaluation index system had certain feasibility and practicality,and provided a scientific basis for the implementation of the comprehensive evaluation of the implementation of smoke-free regulations.It was recommended to carry out further research,to establish more effective data collection channels,to expand the evaluation of urban scope,to continue to improve the indicator system,and finally to establish a scientific and practical comprehensive evaluation index system for the implementation of urban smoke-free regulations.
