ObjectiveDiscussing the theoretical basis of the Rising Qingqi Acupuncture Therapy on respiratory failure with mechanical ventilation. To observe the effect of Rising Qingqi Acupuncture Therapy on the mechanical ventilation and oxygenation in patients with mechanical ventilation, and the effects on the recovery of respiratory function were evaluated. To provide a new way of treatment to improve the successful rate of weaning from mechanical ventilation in critically ill patients.MethodThe 60 patients who compliant with the standard of nanofilm were divided into the trial group and the control group according to the random number table. The control group was treated with conventional western medicine. On the basis of conventional western medicine treatment, the trial group was treated with the treatment of the Rising Qingqi Acupuncture Therapy(Zhongfu,Zhongshan,Hegu,Baihui,Guan Yuan,Zusanli,Lung Yu and Phrenic Shu). The course of treatment was 7 days and the test period was two courses. The individual information of the patients, the history of smoking, the course of primary disease causing respiratory failure, the type of respiratory failure, the combination of multiple organ failure and the state of consciousness were recorded,by comparing the duration of the two groups of patients, the success rate within 14 days, and the oxygenation indexes before and after treatment, third, seventh, and fourteenth days before and after treatment:Oxygenation index(OI=PaO2/FiO2),pH,arterial oxygen partial pressure(PaO2),arterial carbon dioxide partial pressure(PaCO2);infection indicators:leukocyte(WBC),neutrophils percentage(NEUT%),C-reactive protein(CRP);nutritional indicators:hemoglobin(HGB),serum total protein(TP),serum albumin(ALB)Acute physiology and chronic health score(APACHE-?)were used to evaluate the efficacy.IBM SPSS Statistics 25 for MAC was used for statistical analysis.ResultsA total of 60 cases were included in this study, including 30 cases in the trial group and 30 cases in the control group. The trial group dropped 4 cases, completed 26 cases, the control group dropped 3 cases, completed 27 cases. The results of clinical observation and analysis were as follows:(1)Baseline analysis:there was no significant difference between the two groups of patients' age, sex, smoking history, the consciousness state of the inclusion, the course of primary disease, the combination of MODS and the types of respiratory failure(P>0.05),and there was no significant difference in APACHEII score, oxygenation index, infection index, and nutritional index before treatment(P>0.05)the baseline is consistent and has comparability.(2)Mechanical ventilation observation index:the average mechanical ventilation time in the two groups was(252.35±14.44)h in the trial group and(286.51±11.45)h in the control group in 14 days, and the difference was not statistically significant(P>0.05).The successful weaning rates of the two groups were 50%(trial group)and 40.74%(control group)within 14 days, respectively, and the difference was not statistically significant(P>0.05).The total successful weaning rate was 45.28%.(3)Oxygenation index:the oxygen and index of the two groups showed an upward trend before and after treatment, and the difference between the experimental group before and after treatment was statistically significant(P<0.05),and there was no significant difference in the time points of the control group. There was no significant difference in the third day treatment between the experimental group and the control group(P>0.05).The difference between the seventh days and the 14 days was statistically significant(P<0.05).There was no significant change in the abnormal values of the two groups of pH,and there was no significant difference in the changes before and after treatment(P>0.05).PaO2 showed an upward trend, and the difference was statistically significant before and after treatment. There was no significant difference between the experimental group and the control group for third days and seventh days(P>0.05),and there was a significant difference between fourteenth days(P<0.05).PaCO2 showed a downward trend. There was a high statistical difference between the experimental group and the control group before and after treatment(P<0.01),but there was no significant difference between the test group and the control group(P>0.05).(4)Acute physiology and chronic health:two groups of APACHE ? scores showed a downward trend, and the two groups had statistically significant changes before and after treatment(P<0.05).There was no significant difference in the third day treatment between the experimental group and the control group(P>0.05).The difference between the seventh days and the 14 days was statistically significant(P<0.05).(5)Infection index:after treatment, the two groups of CRP,WBC,NEUT%had a downward trend, the changes of CRP and WBC had no significant statistical significance(P>0.05),the NEUT%treatment in the test group decreased significantly(P<0.01),and the difference between the third days before treatment and third to seventh days before treatment was statistically significant(P<0.05).(6)Nutritional indicators:after treatment,HGB,ALB and TP increased, and the difference was statistically significant(P<0.01).There was no significant difference in the change of HGB in the two groups;the improvement of the experimental group after TP treatment was better than that of the control group, and the difference between the fourteenth days was statistically significant(P<0.05);the improvement of the TP in the experimental group was as a whole as the control group, and the difference between the seventh days and the fourteenth days was statistically significant(P<0.05).(7)Mortality:the mortality rate in the trial group was 23.08%within 14 days and 25.93%in the control group. The difference was not statistically significant(P>0.05),and the total mortality rate was 24.53%.(8)Safety evaluation:acupuncture treatment combined with western medicine routine treatment has no obvious adverse reactions.conclusions(1)The Rising Qingqi Acupuncture Therapy combined with traditional western medicine can improve the oxygenation index, improve the hypoxic state, and improve the respiratory function of the patients with mechanical ventilation in respiratory failure.(2)The Rising Qingqi Acupuncture Therapy can reduce the APACHE ? score and improve the acute physiology and slow pathological health of patients with mechanical ventilation.(3)The Rising Qingqi Acupuncture Therapy can cooperate with Albumin to improve the nutritional status of patients with mechanical ventilation.(4)Acupuncture is safe for patients with respiratory failure and invasive mechanical ventilation. No obvious adverse reactions can be seen.It can be further studied in clinical practice.
