Clinical And Mechanism Study Of Yiqi Huayu Jiedu Decoction Combined With Sorafenib In The Treatment Of Primary Liver Cancer

Objective:Based on the real world retrospective analysis of the clinical efficacy of Yiqi Huayu jiedu recipe combined with Solafini in the treatment of primary liver cancer,and the effect of Yiqi Huayu jiedu recipe on apoptosis and autophagy of human liver cancer cell line SMCC-7721 was revealed in vitro.It provides scientific basis for Yiqi Huayu jiedu recipe in the treatment of primary liver cancer.Methods:(1)A total of 50 patients with liver cancer were included in this study.7 cases were lost due to the lack of case data,and 43 cases were finally included.They were divided into Solafini single drug treatment group(control group)and Solafini combined with Yiqi Huayu jiedu recipe group(combined group).The half-year,one-year and two-year survival rates,PFS and adverse reactions during treatment were recorded.(2)Different concentrations of Yiqi Huayu jiedu recipe were used to observe the changes of nucleus in human hepatocellular carcinoma cell line SMCC-7721,by DAPI staining,and the effects of different concentrations of Yiqi Huayu jiedu recipe on cell morphology were observed under microscope.The experiment was divided into four groups: Yiqi Huayu jiedu recipe group,Chonglou saponins I group,rapamicin group,Solafenil group and blank control group.The occurrence of autophagy was observed under confocal microscope,and the autophagy related protein LC-3II,was detected.The expression of p62 and PI3 K,m TOR.Results :Clinical research section:The 1-year,2-year survival rates of the two groups were 95.2% vs 72.7%(P < 0.05),85.7% vs 50.0%(P < 0.05)and 47.6% vs 13.6%(P < 0.05),respectively,and the PFS of the experimental group was 6.99 ±0.60 months and that of the control group was 4.55 ±0.41 months(P < 0.05).Comparison of adverse reaction rates between the two groups: test group VS control group,57.14% vs 59.09%,(P>0.05),the difference was not statistically significant.Experimental research section:DAPI staining showed that Yiqi Huayu jiedu recipe could promote apoptosis and change cell morphology.Besides,Yiqi Huayu jiedu recipe,Chonglou saponins I and Solafinil could block autophagy flow,increase the number of autophagy bodies,up-regulate the expression of autophagy related protein LC-3II,P62 and down-regulate the expression of PI3 K,m TOR.Conclusion:Yiqi Huayu jiedu recipe combined with Solafini can improve the half-year,1-year and 2-year survival rate and PFS;of patients.Yiqi Huayu jiedu recipe can promote apoptosis of liver cancer cells and change cell morphology.Yiqi Huayu jiedu recipe,Chonglou saponins I and Solafinil can block the autophagy flow of HCC cells and increase the number of autophagy,which may be related to the inhibition of PI3K/m TOR signal transduction pathway.In a word,the mechanism of Yiqi Huayu jiedu recipe may be related to its promoting apoptosis and blocking autophagy.
