Objective:Through a rigorous scientific design and a randomized controlled experiment,the effect of heshen pill(HSP)on the expression of CD2AP protein in renal podocytes of rats with adriamycin nephropathy was studied to analyze the relevant mechanism of HSP in the treatment of glomerular disease proteinuria,so as to provide experimental basis for the prevention and treatment of renal proteinuria and the delay of chronic kidney disease.Method:110 healthy and clean male SD rats were randomly divided into HSP group(2.1g/kg/d),enalapril group(1.05mg/kg/d),model group and control group.Adriamycin 6mg/kg was injected into the tail vein once to replicate the model of adriamycin nephropathy in rats.After the successful modeling,HSP group and enalapril group were given corresponding liquid,and the model group and the control group were given equal volume distilled water for 1 day/time for 8 weeks.Urine was collected for 24h at week 2,4,6 and 8,and blood samples from abdominal aorta were collected to detect serum creatinine,serum urea nitrogen and serum albumin.the pathological changes of the kidney were observed under light microscope with HE and Masson staining,and the expression of CD2AP in the kidney tissue was detected by immunohistochemical staining.Result:1.Observation of the general state of ratsRats in the blank group had good spirit,normal activity,defecate,shiny body hair,no shedding,eating and drinking water normally,normal urine and stool,and dry pad materials.The rats in each model group showed different degrees of listlessness,with less movement,less luster and shedding of body hair,less feeding,and stool were not formed.The pad materials were moist and needed to be changed frequently.After the administration,the general conditions of the rats in the HSP group and enalapril group were better than those in the model group,and no significant changes were observed in the general conditions of the rats in the model group.2.Urine protein quantitation for 24hAfter the end of administration,the 24h urinary protein quantification in each model group was significantly higher than that in the control group(P<0.05),and the 24h urinary protein quantification in the HSP group and enalapril group was significantly lower than that in the model group(P<0.05).There was no significant difference in urine protein content between the HSP group and enalapril group(P>0.05).3.Blood biochemical index detectionCompared with the control group,serum albumin in the model group was significantly decreased during the same period(P<0.05).The serum albumin of rats in the treatment group increased at the end of the second week compared with that in the model group,and the difference was statistically significant(P<0.05),but still significantly lower than that in the control group(P<0.05),and close to the level of the control group at the end of the eighth week.Compared with the control group,urea nitrogen and serum creatinine of the model group increased significantly in the same period,and the difference was statistically significant(P<0.05).By the end of the fourth week of treatment,the urea nitrogen and creatinine levels of the rats in the treatment group were significantly lower than those in the model group(P<0.05).There was no significant difference in serum albumin,urea nitrogen and creatinine between HSP group and enalapril group(P>0.05).4.The renal histopathological changes of adriamycin nephropathy rats were observed by light microscopy after HE and Masson stainingIn the control group,the glomeruli were normal,mesangial cells and stroma were not proliferated,and the structure was clear.In the model group,most glomerular volume increased,mesangial cells and mesangial matrix showed focal segmentations,epithelial cell were swollen,abundant inflammatory cell infiltration in renal interstitium,fibrous tissue hyperplasia and protein tubules were observed under light microscope.In the HSP group and enalapril group,the degree of renal tissue lesion in rats was less than that in the model group,with mild mesangial hyperplasia,decreased swollen epithelial cell,and no protein tube type in the renal tubule lumen.5.The distribution and expression of CD2AP protein in renal tissueCompared with the control group,CD2AP protein expression was significantly decreased in the model group(P<0.05).Compared with the model group,the CD2AP level of rats in the HSP group and enalapril group was significantly increased(P<0.05).There was no significant difference in the expression of CD2AP between HSP group and enalapril group(P>0.05).Conclusion:1.HSP can reduce proteinuria and improve renal function in adriamycin nephropathy rats.2.HSP can repair damaged glomerular podocytes,reduce proteinuria and delay the progression of chronic kidney disease by up-regulating the expression of CD2AP.
