[Research Background]Opioid-associated constipation(OIC)is the most common adverse reaction to opioid use, which not only hinders the patient's normal treatment, but also affects the patient's quality of life.At present, the treatment of OIC is mainly based on conservative treatment, including Western medicine treatment and Chinese medicine treatment.Western medicine treatment of OIC mainly includes the use of new drugs such as laxatives, gastrointestinal motility drugs and secretagogues, and physical methods such as enema, and has achieved certain effects, but its treatment is prone to drug dependence and even intestinal nerve damage.Wait for some serious damage.Chinese medicine has a good effect on the treatment of OIC.Traditional Chinese medicine has a good effect on OIC.However, due to the particularity of cancer patients, its gastrointestinal function is fragile.Oral decoction often increases the gastrointestinal burden of patients, but it cannot tolerate.Compared with the internal treatment method,TCM treatment can better protect the gastrointestinal function of patients, avoid the decomposition and destruction of the digestive juice of the upper digestive tract, and avoid the stimulation of the upper digestive tract by the drug, and can directly reach the disease. In the previous clinical work, our instructor has most of the OIC patients with yang deficiency syndrome, using the principle of treatment of Wenyang qi,applying the self-made Wenyang qi qi method to treat the OIC patients with dialectical yang deficiency by using the external treatment of abdominal cream.Have a good effect.[Research purposes]Through the analysis of the treatment of Wenyangyangqifang abdomen cream for patients with dialectical yang deficiency, the clinical value of the patient was evaluated and its effectiveness, safety and scientificity were comprehensively evaluated. This type of patient is provided with an easy-to-accept,safe and effective TCM treatment plan for further clinical promotion, shortening treatment time and improving recovery rate.[research method]Using prospective,open,self-contrast design methods,36 patients with diagnostic criteria collected from May 2018 to March 2019 were included in the study, one of whom fell off halfway and the remaining 35 patients.Through the analysis of the symptom scores before and after treatment, the patient's daily defecation record, the patient's pain score after defecation and the quality of life score before and after treatment, the Wenyang Xingqi abdomen cream was used to treat the tumor OIC with dialectical yang deficiency.Clinical value, comprehensive evaluation of its effectiveness, safety and scientific.[Research result]?Overall efficacy evaluation:For 35 patients who met the inclusion criteria, the changes of constipation scores before and after treatment were evaluated according to the specified efficacy criteria. After statistics,0 cases(0%)were cured after treatment, and 5 cases were markedly effective.(14.3%),19 cases(54.3%)were effective,11 cases(31.4%)were ineffective, and the total effective rate was 68.6%.It is proved that the patients with OIC who have dialectical syndromes of yang deficiency and qi deficiency have a good clinical effect.?Efficacy analysis of TCM symptoms:35 patients who met the inclusion criteria were treated with Wenyangxingqifang abdomen cream. After the end of treatment, according to the changes of TCM symptom scores before and after treatment, the evaluation was based on the specified efficacy criteria.According to statistics,0 cases(0%)were cured after treatment,3 cases(8.6%)were markedly effective,13 cases(54.3%)were effective,19 cases(45.7%)were ineffective, and the total effective rate was 45.7%.It is proved that the treatment of Wenyang qi and abdomen cream can improve the TCM symptoms of the tumor OIC with dialectical yang deficiency.?Analysis of changes in TCM symptom scores:Statistical analysis was performed on the total scores of TCM symptoms before and after treatment in 35 patients,P<0.05.At the same time, the statistical analysis of the scores of 7 TCM symptoms before and after treatment was carried out.The results showed:the frequency of defecation before and after treatment, the degree of defecation, the stool traits, the shortness of fatigue, the coldness of the body, the coldness of the lower abdomen, and the long-term urination. The rank sum test,P value<0.05,was statistically significant, indicating that the treatment has an effect on the symptoms of yang deficiency.?Cartesian score analysis:The Karsten scores of 35 patients before and after treatment were analyzed,P>0.05,the difference was not statistically significant. This suggests that treatment may not improve the patient's physical condition.?pain score analysis:normal examination of KPS score before and after treatment,P>0.05,no statistically significant difference, indicating that the treatment did not relieve the pain symptoms of the patient.?During the treatment, defecation situation:10 patients(28.6%)had no defecation,25 patients(71.4%)had defecation,12 patients had defecation on the first 3 days,22 patients had defecation on the first 5 days and 25 patients had defecation on the first 7 days.?Safety evaluation:There was no serious abnormality in skin-related adverse reactions, blood routine, liver and kidney function, ion level and electrocardiogram level during the whole experiment, indicating that dialectical genus of patients with cancer The secret OIC is a safe and feasible treatment.[Analysis conclusion]The treatment of yang deficiency in patients with cancer is an effective method, which can significantly improve the symptoms of constipation and related TCM symptoms, and is safe and effective. It can be used as an effective method for treating patients with cancer yang deficiency in clinical practice.one.
