Objectives:The pathomechanism and symptoms gastroesophageal reflux disease(GERD)in elderly patients is complicated,and traditional Chinese medicine(TCM)has an advantage in treating this condition.This study aims to explore the characteristic TCM syndromes of this condition through statistical analysis of available literature,epidemiology,disease type and TCM syndrome study,so as to shed light on the treatment of GERD in elderly patients using TCM.Methodology:A total of 203 cases of elderly GERD patient cases were collected.General information,TCM syndrome information,results from endoscopy were collected to form a data bank.SPSS 24.0 was used for statistical analysis.Descriptive statistics and inference method were used to analysis the general information,factors causing the onset of disease and the disease type.Factor analysis was used to explore the TCM syndrome and characteristics of this condition.Results:1.General information:The 203 elderly GERD patients were aged 60-85(68.98±6.512).87(42.86%)of the patients were male and 116 were female(57.14%),the ratio of male to female is 0.75:1.The BMI distribution of participants were as follow:Normal at the highest with 44.83%,overweight at 39.41%,obese at 9.36%,and underweight at 6.4%.71%of the patients had at least middle school education.In terms of dietary preference,prefer deep fried,oily and high fat food(22.17%)>Spicy and stimulating food(20.2%)>Food that are too high in temperature(18.72%)>strong tea(15.27%)>high salt diet(14.29%)>preserved food(11.33%)>coffee(3.94%)>sweets(3.45%)>sour food(2.96%)>food that are cold(2.46%)>carbonated drinks(1.48%).In terms of unhealthy dietary habits,irregular meals(18.72%)>food intake 2 hours before sleep(17.24%)>too little intake(11.33%)>overeat(5.42%),56.16%of patients had good eating habits.In terms of alcohol intake and smoking history,29.06%reported a history of smoking,17.24%had history of drinking;smoking and drinking each accounted for 62.07%and 36.78%of study participants respectively.Psychologically,60.59%were abnormal psychologically,of which quick-temper(33%)>anxious(30%)>depressed(13.3%)>psychological trauma(1.97%)>easily startled(1.48%).2.Disease type:RE 112 cases(55.17%)?NERD 85 cases(41.87%)?BE 6 cases(2.96%).Within RE,there were LA-A 70 cases(34.48%)?LA-B 27 cases(13.3%)?LA-C 10 cases(4.93%)?LA-D 5 cases(2.46%).In terms of disease type and its association with gender and age,within male cases,RE(66.66%)>NERD(28.74%)>BE(4.6%);female cases,NERD(51.72%)>RE(46.56%)>BE(1.72%).The percentage of NERD is highest at 51.35%for the age group 60-69,and RE is most prevalent at 64%for age group 70-79.The prevalence of NERD cases decreases with age,while it increases with age for RE cases,of which the pattern of increase in prevalence is most obvious in LA-A and LA-B.Concomitant disease observed under endoscopy includes:GERD combined with hiatal hernia 51 cases(25.12%),GERD combined with chronic atrophic gastritis 46 cases(22.66%),combined with bile reflux 32 cases(15.76%).3.TCM syndrome:Of all the symptoms,acid reflux(79.8%)was the most prevalent followed by heartburn(76.4%).The TCM location of the disease is primarily at stomach and liver,and is closely related to spleen,gall bladder,large intestine and kidney.The primary full syndrome includes Qi stagnation and heat,and it is closely associated with phlegm,dampness,blood stagnation,food accumulation(indigestion)and excessive yang.The primary deficient syndrome includes Yin deficiency and blood deficiency,and is closely associated with Qi deficiency,yang deficiency and Essence depletion.Conclusion:1.Prevalence of GERD in elderly patients is higher in females than males.The onset of the disease is associated with alcohol intake and smoking history and psychological state.The association with BMI,dietary preferences and eating habits are inconclusive.2.There is a statistical difference in the distribution of gender and age of the disease type in elderly GERD patients.Male patients primarily suffer from RE while female patients suffer from NERD.The proportion of NERD patients decreases with age while the proportion of RE increases with age,with significant pattern seen in prevalence of LA-A and LA-B RE.3.Elderly GERD patients primarily display acid reflux and heartburn symptoms.The TCM disease location is primarily at stomach and liver,and is closely associated with spleen,gall bladder,large intestines and kidney.The primary full syndrome is qi stagnation and heat,and is associated with phlegm,dampness,blood stagnation,food accumulation(indigestion)and excessive yang.The deficient syndrome is primarily yin deficiency and blood deficiency,and is associated with Qi deficiency,Yang deficiency and Essence depletion.
