Analysis And Study Of The Clinical Tongue Picture In The Treatment Of Chronic Venous Edema Of Lower Extremities

Chronic lower extremity venous edema is one of the most common symptoms of chronic venous diseases of the lower extremities.Chronic venous edema of the lower extremities will be followed by thrombophlebitis,deep vein thrombosis,dermatitis(eczema,psoriasis,neurodermatitis).,stasis dermatitis,atopic dermatitis,skin amyloidosis),refractory ulcers,even amputations or severe infections are life-threatening.Therefore,chronic venous edema of the lower extremities is often evaluated as a special disease that seriously affects the patient's life and survival treatment,and is one of the most common and difficult diseases in the specialty.Traditional Chinese medicine has obvious advantages in the treatment of chronic lower extremity venous edema,and tongue diagnosis is one of the most important basis for clinical syndrome differentiation.Its status is crucial.Therefore,if we can summarize the characteristics of tongue in patients with chronic venous edema of the lower extremities,it is of great significance for guiding the diagnosis,treatment and medication of chronic venous edema of the lower extremities,judging the prognosis of outcome and secondary syndrome.Objective:(1)To determine the common features of patients with chronic venous edema of the lower extremities in tongue diagnosis;(2)Verification of the characteristics of the pathogenesis of chronic venous edema of the lower extremities;Methods:200 patients with chronic venous edema of the lower extremities diagnosed from the outpatient clinic of Beijing University of Traditional Chinese Medicine and Dongzhimen Hospital of Beijing University of Chinese Medicine from May 2015 to December 2018 were studied.Collect the patient's basic medical history and tongue image data,invite experts to identify and compare,and descriptively analyze the distribution characteristics of different tongue images.Results:(1)Among 200 patients with chronic edema of lower extremity,the pale red tongue accounted for 13.50%,the red tongue accounted for 15.50%,the tongue tip red ratio was 2.00%,the tongue accounted for 10.50%,and the pale purple tongue accounted for 17.00%.Purple tongue accounted for 24.50%,purple tongue accounted for 17.00%;(2)Among 200 patients with chronic venous edema of lower extremity,fat tongue accounted for 56.50%,skinny tongue accounted for 20.50%,moderate tongue accounted for 23.00%;Cang old tongue accounted for 26.00%,delicate tongue accounted for 27.00%,moderate The tongue accounted for 47.00%;the tooth mark tongue accounted for 50.50%,the crack tongue accounted for 30.50%,and the point tongue accounted for 23.50%;(3)Among 200 patients with chronic venous edema of lower extremity,white moss accounted for 72.00%,yellow moss accounted for 24.50%,and gray black moss accounted for 3.50%;(4)Among 200 patients with chronic venous edema of lower extremity,the proportion of thick moss was 30.00%,the proportion of thin moss was 49.00%,the proportion of moderate moss was 21.00%,the proportion of dry moss was 14.50%,the proportion of moist moss was 72.00%,and the proportion of moderate moss was 13.50%,greasy moss accounted for 62.00%,spoiled moss accounted for 9.00%,moderate moss accounted for 34.00%;stripped moss accounted for 4.50%,partial moss accounted for 14.00%,total moss accounted for 81.5%;(5)200 patients with chronic venous edema of lower extremity,according to the degree of edema,59 cases of edema,accounting for 29.50%,74 cases of second degree edema,accounting for 37.00%,67 cases of third degree edema,accounting for 33.50%.In one degree,there were 22 cases of fat big tongue in edema,accounting for 19.47%,46 cases of second degree edema and fat tongue,accounting for 62.16%,and 45 cases of third degree edema and fat third degree edema,accounting for 62.69%;(6)Among 200 patients with chronic venous edema of lower extremity,according to CEAP classification,there were 92 cases of C3 grade,accounting for 46.00%,59 cases of C4a grade,accounting for 29.50%,and 49 cases of C4b grade,accounting for 24.50%;There were 44 cases of fat tongue in C3 grade,accounting for 47.83%,36 cases of fat tongue in C4a grade,accounting for 48.65%,33 cases of fat tongue in C4b grade,accounting for 49.52%;The number of tongues was 19 cases,accounting for 32.20%,the number of teeth in the C4a class was 43 cases,accounting for 58.11%,the number of teeth in the C4b class was 33 cases,accounting for 58.21%;the number of cracked tongues in C3 was 6 cases,accounting for Compared with 6.52%,there were 29 cases of cracked tongue in C4a grade,accounting for 49.15%,26 cases of cracked tongue in C4b grade,accounting for 38.81%;14 cases of middle point of C3 grade,accounting for 15.22%,in C4a grade There sublingual There were 21 cases of varicose veins,accounting for 35.59%,and 26 cases of sublingual collateral varices in C4b grade,accounting for 53.06%;Conclusion:Tongue characteristics of 200 patients with chronic venous edema of the lower extremities:tongue color is mainly caused by bloody tongue-related tongue color such as purple tongue and purple tongue;tongue shape is mainly related to spleen deficiency and wet resistance,such as scallop tongue and fat tongue;Moss is mainly dominated by normal mossy white moss;as the degree of edema continues to increase,the proportion of fat tongue and scalloped tongue also increases,indicating the close relationship between the pathogenesis of edema and spleen deficiency;The progress of chronic venous edema of the lower extremities,the proportion of fat tongue and dentate tongue,the number and proportion of sublingual varices are also on the rise,indicating the closeness of spleen deficiency,blood stasis and chronic venous edema of lower extremities.Relationship;the increase in the proportion of cracked tongue and point tongue also indicates that yin deficiency and blood deficiency are also factors that cannot be ignored in the development of chronic lower extremity venous edema.
