Objective:The previous study has clinical effect on single supporting splint fixation for the treatment of children with extension type supracondylar fracture of humerus were observed and confirmed the clinical reliability of splint fixation for the treatment of orders after the given mechanism on the stability and there are still some puzzling questions,this research intends to observe after comparing splint and four support a splint,combined with modern mechanics analysis method(finite element analysis)to elucidate the mechanism of action of the single supporting splint fixation for the treatment of.Methods: From August 2015 to March 2018,with 60 cases of children with type supracondylar fracture patients included in the standard according to the date of admission were randomly divided into 2 groups,30 patients in each treatment group,after the restoration,using the single supporting splint after rotation about 120 degrees of elbow flexion fixation,control group with four in one clamping plate after the elbow.About 90 degrees,respectively,were observed after the treatment the carrying angle changes.At the same time,through a CT scan of a normal upper limb,the image input Mimics 13.1 single after fixation for the treatment of humeral supracondylar fracture model,the model is imported into ANSYS 10 software for finite element analysis,the stress and displacement observation changes of elbow joint atdifferent flexion angles and loads to analyze the stability of splint the support list.Results :After operation in the 2 groups between the groups in the change of the carrying angle differences were not statistically significant(P>0.05);found in three different tests,the elbow joint in 120 degrees of flexion and supination,the broken end of the minimal stress fracture;elbow supination in 120 degrees of flexion.The fracture broken ends of strain minimum;elbow joint in 120 degrees of flexion and supination,fracture displacement is minimum.Conclusion: 1.in the clinical observation,by comparing the single support before and after the splint and four splint fixation for the treatment of limb carrying angle change,the two fixation methods with satisfactory results.2.the clinical curative effect of single posterior splint fixation is consistent with the mechanics mechanism shown by finite element analysis.3.finite element analysis showed that the elbow supination in 120 degrees of flexion,the maximum stress in elbow humeroradial joint,radius from the front of the extrusion force of the capitellum is equivalent to a front splint,this may constitute a single factor after the trust does not need the front splint splint.4.at the same time,through the finite element results are fixed on the three different aspects of the data analysis show that this kind of fixed mode in rotationafter the elbow 120 degrees for the optimal steady state position,also consistent with clinical experience.
