Objective: To study the treatment of open reduction and cross-pinning of supraco- ndylar humerus fractures (Gartlandâ…¢type) the clinical efficacy and axes in order to restore the principle of fracture fragments, carrying angle adjustment of surgery to prevent the incidence of cubitus varus, cubitus lower turn the feasibility and effectiveness of the incidence analysis. Methods: 72 patients met the inclusion criteria selected children treated supracondylar fractures (Gartlandâ…¢type) patients, according to the different treatment methods are divided into A group and B group, A group of 50 patients choice of treatment with open reduction and cross-lateral incision g Stipa fixed; B group, 22 cases were treated with C-arm fluoroscopy closed reduction and percutaneous pinning fixation. Two groups were gender, age, swelling, distribution of disease location, fracture type of comparison, there was no significant difference between comparable. After two groups of patients were carried on the angle, elbow flexion and extension activities, functions, the incidence of cubitus varus and overall outcomes were compared to evaluate the treatment of open reduction and cross-pinning of supracondylar humerus fractures fixed (Gartlandâ…¢type) clinical efficacy, and further analysis of the indications for surgical treatment of open reduction, and analysis to bring the healthy side of patients based on the size of angle in order to restore the principle axis of fracture fragments, carrying the angular size of the adjustment operation in the prevention of cubitus varus role.Results: 72 patients were completely healed fracture without Volkmann ischemic contracture, myositis ossificans and serious vascular complications such as nerve damage. Group B was due to carry degree angle to reduce group A, group B in the prevention of proof of cubitus varus was superior to A group, A group and B group after the final follow-up activity in the elbow area, there was no difference by statistical analysis, that the two groups can achieve good results. According to Flynn evaluation criteria: A high rate of treatment group 72.73%, B group after treatment, excellent rate of 92.20%, the Z test, Z =- 3.40, P = 0.01, P <0.05, significant difference. Showed that after treatment B (open reduction and internal fixation) was significantly higher than that of good A (closed reduction and percutaneous Kirschner wire fixation).Conclusion: 1. Open reduction and Kirschner wire fixation of supracondylar humerus fractures (Gartlandâ…¢type) is simple, satisfactory reduction, shorter operative time, there is no reset operation repeatedly to reduce the loss of the elbow, and fixed firmly, good recovery of elbow function, low incidence of cubitus varus with closed reduction and percutaneous Kirschner wire fixation greater than the advantages for the surgical treatment of children with supracondylar fractures (Gartlandâ…¢type) treatment of the corresponding reference . 2. Closed reduction and percutaneous pinning of supracondylar fracture of humerus in children (Gartlandâ…¢type) are less invasive and leave no scar characteristics, but the incidence of cubitus varus is still high, increasing the risk of orthopedic surgery again. 3. To carry angular size of the healthy side as a reference, recovery of patients with fracture fragments of the bit lines, thorough, correct ulnar deviation, and rotation around the principle of displacement, suffering from elbow injury before the resumption of the normal axis to restore satisfactory carrying angle the purpose of the treatment concept, GartlandIII type supracondylar humerus fractures is important. Therefore, the reduction of supracondylar humeral fractures (Gartlandâ…¢type) the incidence of cubitus varus effective way to correct a good alignment of the line and firmly fixed.
