Observation On The Clinical Efficacy Of External Application Of Guyan Ointment Combined With Intravenous Drip Of Mannitol In The Treatment Of Closed Femoral Shaft Fractures And Soft Tissue Swelling And Pain (stagnation Of Qi And Blood Stasis)

Objective: The purpose of this research is to observe and compare the therapeutic effects in soft tissue which is swelling and aching in closed fracture of femoral before the operation by using Gu Yan Gao,Sodium Aescinate respectively on the basis of Mannitol treatmentMethods: 87 patients with femora fracture are involved in this study.they stay in our hospital from June 2016 to October 2017 and all met the diagnostic criteria of such disease.they were randomly divided into three groups by SPSS software according to the admission sequence: group 1(Gu Yan Gao combine Mannitol)group 2(Sodium Aescinate combine Mannitol)and control group(Mannitol).Group 1 were treated with Mannitol and covered with Gu Yan Gao on affected parts and bandage it appropriately.Group 2 were treated with Mannitol and covered with Sodium Aescinate on affected parts and bandage it appropriately.Group 3 were treated with Mannitol.They treatment period last for 7 days.Observational index includes blood Hemorheology before and after treatment respectively: whole blood viscosity hematocrit,fibrinogen;serum inflammatory cytokines:TNF-α,LI-6;soft swelling rate of patients,pain relief time for the first dermatoglyph,tension blisters,allergic dermatitisResult:they were no significant differences between the three groups before the treatment(p>0.05)during therapy period,allergic dermatitis and Phlebitis of group2 and group3 are 2 case and 1 case respectively.The analysis of hemorhe ology:the whole blood viscosity,hematocrit,fibrinogen index in blood rheology of three groups are statistically significant before and after treatment Inter-g roupcomparison:experimental groups(1,2)are lower than control group,group 1 is lower than gropu 2 which mean they are all statistically significant.serum inflammatory factors:three groups are all statistically significant before and after treatment.But group 1and group 2 have no statistically significant before and after treatment.Swelling rate:the swelling of each group showed an upward trend during the treatment period from day 1 to day 3.the rising range of swelling of experimental groups are lower than the control group and the rising range of group 2 is lower than group 1.which shows they all statistically significant.From 5th to 7th day.the falling range of swelling rate experimental groups was higher than control group.group1 was higher than group 2,which means the differences are all statistically significant.Pain relief rate: the difference between experimental groups and the control group was statistically significant.Time for the first dermatoglyph: the difference between experimental groups and the control group was statistically significant,but difference between group1and2 have no statistically significant.Conclusion: Gu Yan Gao can effectively relieve the soft tissue swelling and aching closed femora fracture.it can obviously improve the early blood rheology and inflammatory response.Results show that treated with combination of Gu Y an Gao and Mannitol has batter therapeutic effect than single treatment.there fore,Gu Yan Gao combined with Mannitol is an effective method in treating soft tissue swelling and aching in closed femoral fracture before operation.
