An Analysis Of A Successful Case Of Mandibular Comminuted Fractures After Infection Using Mini-titanium Plate Internal Fixation And Using Reconstruction Titanium Plate

Objective: Mandible is an important organ in oral and maxillofacial region,which plays an important role in maintaining the shape of the jaw,chewing,breathing and so on.With different ways of reconstruction and repair of mandibular defects in the research,has been the focus at home and abroad of oral and maxillofacial surgery workers pay close attention to,and in recent years mini titanium plate in mandibular fracture are widely used,the fracture of mandible reconstruction into a new stage of development,but its strength is not enough,solid power shortage has been puzzling clinic application problem.This experiment through a detailed record of 1 cases of mandibular fracture patients after failure repair of titanium mini plate use reconstruction titanium plate reconstruction surgery after,through analysis and summary of the mini titanium plate and titanium plate reconstruction surgery to disease,contraindication and postoperative complication extremely prevention.Methods: Results in patients with mandibular fracture by 2012 were retrospectively analyzed a case of injury in traffic accident,the patient due to the trauma of the accident in the outer court with titanium mini plate internal fixation for the treatment of mandibular fracture,postoperative recovery is poor,wound infection and the surrounding bone necrosis into our hospital with reconstruction titanium plate made good curative effect in the restore the shape of the mandible.Through the case analysis,to explore in the rigid internal fixation of mandibular fracture should choose titanium plate,avoid the risk of surgery and reduce the operative failure.Results: The patient through the operation with reconstruction titanium plate internal fixation of mandibular,intraoral occlusal relationship good recovery,mouth opening recovery normal,left submandibular fistulous tract disappear,the patient is satisfactory after operation.Conclusion: 1.The stability of mini titanium plate is low,and the resistance to torsion is poor.However,it is widely used to repair all types of mandibular fracture except mandibular fracture with fracture and comminuted fracture.2.the reconstruction of titanium plate fixation,suitable for bone defect with mandibular fracture and multiple comminuted fracture of the mandible.In the patients with bone defect of the mandible fracture of the pillar is fixed,in crushing mandibular fracture across the fracture splintered region carrying all the retention force.3.for the treatment of mandibular multiple,comminuted fracture,surgery can be with mini titanium plate fixed,then the retention force stronger reconstruction titanium plate across the comminution of the fracture zone,thereby increasing the fixed continuous improve the stability.4.in order to facilitate better control of the mandibular width in the operation of the proposed 2.4 mm reconstruction plate to strengthen the fixed,can achieve a more ideal control effect.
