1.Objective:Knee Osteoarthritis(OA)is a common chronic musculoskeletal disease,and pain is the primary symptom and result of complex interactions between structural changes,physical injuries,and psychological factors.Exercise is considered as the main way to improve knee OA,and strengthening lower limb muscle strength is a more effective means of exercise intervention.Studies have found that different exercise patterns can produce different curative effects,so it is of great significance to explore the optimal exercise pattern for the improvement of knee OA by exercise intervention.The purpose of this study was to investigate the effect of slow centrifugation and slow centripetal training on knee OA.2.Methods:Fifty female patients with knee OA were randomly divided into slow eccentric training group and slow concentric training group.Both groups trained for 8 weeks,twice a week,with 10 repetitions in 3 groups.The height,weight,blood pressure,heart rate,fat loss weight,muscle mass of lower limbs and WOMAC osteoarthritis index of the subjects in the slow eccentric and slow concentric training group were measured at the same time three days before the training and two days after the last training.Body function(20m time walking test,6min walking test,sitting and standing test,step test),joint range of motion,single and double leg 1RM and knee extensor torque test were performed.3.Results: 3.1 The changes of WOMAC osteoarthritis index in the slow eccentric training group and the slow concentric training group were as follows :(1)compared with the 8 weeks before the training,WOMAC pain values of subjects in the slow eccentric training group and the slow concentric training group were significantly decreased(P < 0.05).There was no significant change in WOMAC stiffness(P > 0.05).WOMAC body function values were significantly decreased(P < 0.001).(2)after 8 weeks of training,there was no significant change in WOMAC osteoarthritis index between the slow eccentric training group and the slow concentric training group(P > 0.05).3.2 The body shape parameters of the subjects in the slow eccentric and slow concentric training groups were as follows:(1)compared with before 8 weeks of training,fat loss weight of subjects in the slow eccentric training group and the slow concentric training group was significantly increased(P < 0.05),and muscle mass of lower limbs was significantly increased(P < 0.01).(2)after 8 weeks of training,there were no significant changes in fat loss weight and lower limb muscle mass between the slow eccentric training group and the slow concentric training group(P > 0.05).3.3 The changes in body function parameters of the subjects in the slow eccentric and slow concentric training groups were as follows :(1)compared with 8 weeks before the training,the sitting and standing tests of subjects in the slow eccentric training group and the slow concentric training group were significantly reduced(P < 0.05)and the step test were significantly increased(P < 0.05).There was no significant change in 20 m timed walking test and 6min walking test(P > 0.05).(2)after 8 weeks of training,there were no significant changes in the sitting-and-standing test,step test,20 m timed walking test,and 6min walking test between the slow eccentric training group and the slow concentric training group(P > 0.05).3.4 The changes of muscle strength in one leg of subjects in the slow eccentric and slow concentric training groups were as follows :(1)compared with the 8 weeks before the training,the 1-rm values of extensor genu,flexor genu,abductor hip and extensor hip muscles of subjects in the slow eccentric and concentric training groups were significantly increased(P < 0.01).(2)after 8 weeks of training,there was no significant change in 1-rm values of extensor genu,flexor genu,abductor hip and extensor hip muscles between the slow eccentric and slow concentric groups(P > 0.05).3.5 The changes of the maximum muscle strength and muscle moment of the subjects in the slow eccentric and slow concentric training group were :(1)compared with before 8 weeks of training,the maximum muscle strength of the subjects in the slow eccentric and slow concentric training group was significantly increased(P < 0.01).All the extensor genu torques increased significantly(P < 0.05).(2)after 8 weeks of training,there was no significant change in the maximum muscle strength of both legs and extensor moment of knee in the slow eccentric and slow concentric training groups(P > 0.05).4.Conclusion: 4.1Slow eccentric and concentric training improved the pain symptoms of middle-aged and elderly women with knee osteoarthritis.4.2 Slow eccentric and concentric training improved the body function of middle-aged and elderly women with knee osteoarthritis.4.3 Slow eccentric training has the same effect as slow concentric training,which is related to the enhancement of muscle mass and strength.
