As a chronic lifelong disease,diabetes has gradually become the focus of the whole society.People with diabetes not only bear physical and mental pain,but also have to prevent complications.This requires patients with diabetes not only to use drugs scientifically,but also to plan and control themselves reasonably,so as to combat the disease.Therefore,it is imperative to learn scientific self-management!Based on the actual needs of patients with "new sugar" in B hospital,social workers use the group work to intervene,which has a certain practical meaning.Under the guidance of cognitive behavior theory,this paper collects data through interviews and questionnaire to understand the current situation and actual needs of "new sugar" patients' self-management.In the group activities,the group members are divided into two groups,the experimental group and the control group.The experimental group held activities once a week according to the needs of the group,which had a total of four group activities,with the help of diabetes self-management ability scale for comparison and analysis.The study found that the self-management ability of patients with "new sugar" in the experimental group was significantly improved,and the intervention effect was significant.The framework of this paper is split into six parts.the first is the introduction,which mainly studies the research background,research origin,literature review,research purpose and significance and related concepts.The second chapter is the research design,which mainly introduces the research problems,research objectives,research ideas,research methods,object selection and related research theories of this project.The third chapter mainly discusses the current situation of post-hospital self-management of new sugar patients.The fourth chapter is the group intervention in the post-hospital self-management practice of new sugar patients,mainly introduces the group objectives,member recruitment,group process and group summary.The fifth chapter is the evaluation of group practice,which mainly from the satisfaction of members,the process of the group and the effect of thegroup.The sixth chapter is the research summary and related countermeasures.
